Lung surgery can be done in two ways. These; closed method surgeries and open surgeries. The patient's condition and the type of disease shed light on which method is appropriate.
What are the Lung Surgery Methods?
The lungs are a pair of organs located inside the thorax and surrounded by the diaphragm at the bottom and the ribs at the front. The inhaled breath goes to the lungs through structures called trachea and bronchi. The space between the lungs and the thorax is called the pleural space. The right and left lungs consist of lobes. There are two lobes in the left lung and three lobes in the right lung. Lung diseases sometimes involve all lobes and sometimes certain regions and lobes of the lung. Therefore, in some cases, a large part of the lung and in some cases the relevant lobes must be removed in lung surgery.
(ETS) is an endoscopic Closed Lung Surgery method. In this method, the Endoscopic Camera is advanced into the chest cavity through 1,2 or 3 1 cm incisions made between the ribs. The image is projected onto the video screen, special surgical instruments are advanced through the incisions and the procedure is performed. The most recent endoscopic method in Closed Lung Surgery is Single Port VATS. In this method, the procedure is performed by entering through a 2 cm incision. Patient comfort and cosmetic results are very satisfactory in Single Port VATS.
Other Closed Lung Surgery Thoracoscopy is preferred for patients who cannot handle open surgery and have low respiratory capacity. In thoracoscopy, a special device called a thoracoscope is used during the surgery. This device is a metal tube with an optical system that provides illumination and examination of the pleural cavity. In the closed method, the thoracoscope is inserted into the chest cavity through an incision made between the two ribs. Thus, the removal of adhesions that may occur in the pleura and the collection of water around the lungs are prevented. With this method, it is also possible to take tissue for use in histological examination.
Closed Lung Surgery has some advantages. The main ones are the short duration of the operation and less post-operative pain. Complications and hospital stay of these surgeries are also less. No intensive care is required after surgery. The patient has a normal life in a shorter time. returns to his horse.
Lung cancer and tumors, cysts, mediastinal tumors can be operated with the Closed Method. Again, lung biopsy, evacuation of the fluid accumulated in the chest cavity, diaphragm surgeries are also treated with closed method.
In Open Lung Surgery, a large incision is made in the chest (thoracotomy). Lungs can be seen directly during surgery. With this method, some or all of the lungs can be removed. In these surgeries performed under general anesthesia, the patient is placed on his side after anesthesia and an incision is made under the scapula. Through this incision, the lungs are reached between the ribs and the surgery is performed. Complications may occur after surgery. The most common complications are; Infection is a blood clot in the lungs or legs, leakage of air from the lungs. Open Lung Surgery is usually performed if a mass is detected in the lungs, taking into account the size and location of the mass. If the mass is found to be cancer, some or all of the lungs may need to be removed.
