Borderline Vortex: Borderline Personality Disorder

SHEMA THERAPY EFFECT ON BORDERLINE VIRTUAL

Borderline Personality Disorder is a very difficult disorder to treat. It was first started to be treated with Analytical Therapies, then Cognitive Behavioral Therapy was tried. However, no therapy system has been as effective as Schema Therapy developed by Jeffrey Young. According to the researches; In psychoanalytic therapy, discontinuation from therapy (46-67%) and suicide attempts were found to be quite high. In a study conducted in Norway, 50% of the percentage of schema therapy treating borderline patients were no longer diagnosed as borderline; It has been observed that 80% benefit greatly from treatment.

What is Borderline Personality Disorder? (There must be at least 5 items from DSM 5 diagnostic criteria)

1. Making intense efforts to avoid real or imaginary abandonment (excluding suicide and self-harm attempts to avoid abandonment) (abandoned child schema- hurt child mode)

2. Having a two-pronged interpersonal relationship cycle, such as praising the other person at once, glorifying them and putting them down at the same time (inconsistency in emotional reactions due to rapid transitions between modes.)

3. Experiencing identity confusion. Having a distinct and constantly inconsistent, fluctuating identity (identity confusion due to rapid transitions between modes)

4. Impulsivity in at least 2 areas of self-harming (eg: spending a lot, promiscuous sex, substance use, dangerous driving, binge eating...) (suicide and self-mutilating behaviors (impulsive-angry child mode)

5. Repetitive suicidal behaviors, attempts, threats, or self-mutilation (eg razing...) (punishing child mode)

6. Affective inconsistency due to impulsivity in mood (eg, cycle of intense periodic dysphoria, irritability, or boredom that usually lasts longer than a few days) (mode cycle)

7. Persistent sense of emptiness (detached guarded mode)

8. Inappropriate intense anger or controlling one's anger Having difficulty in acting (frequent anger, constant anger, frequent fights) (angry-impulsive child mode)

9. Temporary skeptical thoughts or signs of severe dissociation related to the compulsion ( detached sheltered mode)

So what are Borderline's triggered schemas and modes?

Active schemas of people with borderline personality:

“-abandonment schema

-skepticism-abuse schema

- emotional deprivation schema

-imperfection-shame schema

-addiction-insufficiency schema

-invulnerability schema to illness or harm

-inadequate self-control- self-discipline schema

-submissiveness schema

-emotion suppression schema

-punishment schema” (Arnoud Arntz and Hannie van Genderen “borderline personality disorder) ” p.30)

J. Young developed 'Mod Therapy' as the transition between these schemas, which are active in the borderline, and the mood changes very quickly. The concept we call mode is the momentary mood that occurs when the schemas are triggered. In order not to complicate the already complex structure of Borderline, mod therapy is applied.

Borderline's Mode Cycle:

Individuals with Borderline Personality Disorder They do not consistently display angry, angry, impulsive and self-harming behaviors in the DSM 5 diagnostic criteria. They usually live in a detached sheltered mode, withdrawing into themselves in order to maintain their own spiritual integrity. In this process, they try to stay away from emotionality and engage in activities that console them, eg excessive sex, substance use, excessive eating, excessive book reading, excessive work, etc. when he senses any imaginary threat, he experiences intense pain in abandoned-hurt child mode. Afterwards, she harbors anger at those who upset her so much. The intense anger and impulsiveness he has experienced causes his behaviors that he actually does not want to do(angry-impulsive child mode). An inner soul questioning how she did these things after the anger she had unleashed. the sound causes him to punish himself (punitive parent mode). The person may want to commit suicide in the extreme to punish himself. It can then revert back to its detached sheltered mode. And the cycle continues. In order for the person to break this cycle, it is necessary to strengthen the 5th mode, the healthy adult mode.

Therapists, with the help of schema therapy, help the person to strengthen his/her healthy side, to express his/her emotional needs and to meet his/her own emotional needs. helps him discover ways and integrate them into his life.
