Aesthetic Nose Surgeries

“RHINOPLASTY OR SEPTORINOPLASTY”

Surgeries aimed at changing the external appearance of the nose are called Rhinoplasty. Since these surgeries are often performed together with the correction of internal nasal bone and cartilage curvatures (septum deviation), they can also be called "Septo-Rhinoplasty".

The nose is an important aesthetic element located in the middle of the face. Although perceptions and understandings about aesthetics and beauty may vary from person to person, race, society and period, there are still some common perceptions about beauty. The most important element for the nose is the proportions and symmetry of this structure, located in the middle of the face, in relation to the other parts of the face. The perception of symmetry in any object is a common reason for appreciation. Today's aesthetic nose approach is "naturalness". The common expectation of people who want to have this surgery is that it should be natural and beautiful. However, this word is not descriptive enough. The person who decides on such a surgery must be able to define and express himself very well, explain what he wants, and understand very well what can be done for him. After all, this is not play dough and it is not possible to transform any nose into any desired nose with surgery. The patient and the doctor should be able to explain and understand each other very well what they mean by naturalness and beauty. It is very important that the patient's expectations match the doctor's actions before the surgery, that is, the expectations can be brought to the same level. In this way, any possible disappointments will be minimized. Today's equivalent of an aesthetically successful nose surgery is a nose that suits the person's facial proportions, complements and reveals the beauty of the face, but does not reveal that the surgery has been performed at first. Of course, it may not always be possible to achieve this goal. The physician should not promise unnecessary hopes or sell dreams to his patient. It should not be forgotten that aesthetic nose surgeries are completely personalized surgeries. Since everyone's nose and facial structure is different, the work that can be done is different for everyone and the results to be achieved are also different. After all, none of us are just noses. The changes made will not only suit the person's face, but also their psychology, personality and mood. It is also important that it suits the job and social environment. The doctor must know the patient in all aspects and decide on the surgery to be performed accordingly. In fact, one must sometimes decide that it is better not to perform surgery and be able to explain this to the patient. So, in some cases, not performing surgery may be much more beneficial, both aesthetically and psychologically. On the other hand, it may be much more difficult to achieve the desired results in people who have had an accident or other nose surgeries before.

There are three important elements that define success in rhinoplasty (or septo-rhinoplasty) surgeries:

  • Functionality: The nose is an organ whose main function is breathing. In addition, it also has functions such as humidifying, filtering, heating and smelling the air passing through it. In other words, your nose should breathe well and in high quality after the surgery, and these functions should not be impaired.
  • Beauty: If the patient likes his nose after all the effects of the surgery have passed, it means that he has achieved his goal.
  • Durability: The nose must be strong and stable. There should be no deterioration such as falling, bending or collapsing after a while.
  •

    CLOSED AND OPEN RHINOPLASTY:

Closed or open technique is commonly used for rhinoplasty surgery today. Which technique will be used in your surgery depends entirely on the success achieved. In other words, whichever technique will give better results should be chosen.

    CLOSED TECHNIQUE RHINOPLASTY:

In order to perform rhinoplasty surgery, certain areas of the nose must be cut. In the closed technique, all incisions are inside the nose. There is no incision or scar visible from the outside. Surgery time is usually slightly shorter than the open technique. Post-operative swelling resolves in a shorter time.

    OPEN TECHNIQUE RHINOPLASTY:

In the open technique, a horizontal incision is made on the front nasal pillar that separates the two nostrils. Other incisions remain inside the nose in connection with this external incision. In other words, there will be a 3-4 millimeter incision scar in front of the nose. This scar usually becomes invisible in 2-3 months. A

    SURGERY CONDITIONS AND DURATION:

Rhinoplasty surgery should be performed under full-fledged hospital conditions and under general anesthesia. any In case of a medical complication, there should always be a team and equipment (specialist doctors from all branches and necessary medical devices) in the hospital that can solve this problem.

Before the surgery, some tests are performed and the patient is evaluated by the anesthesiologist to see if there is any harm in having surgery. determines. Patients with any other additional health problems should be examined and evaluated by the relevant branch in advance (such as lung, heart, blood pressure, liver, kidney and thyroid diseases...) There should be no active febrile or flu disease during the surgery period, blood thinners such as aspirin and vitamin E should be taken. Medicines should not be used. If used, it should be discontinued at least a week in advance. The patient is taken to the operating room with a serum attached and a relaxing medication administered.

The duration of the surgery may vary between one and three hours. While the surgery takes about an hour with the closed technique in a patient who has surgery for the first time, this period may take up to 3 hours in patients who have had an accident or surgery (revision) before.

Cartilage tissue may be needed to repair some areas during the surgery. These pieces are taken from the patient's own inner nasal cartilage. If there is not enough cartilage in the nose, it can be taken from the auricle or rib. If this possibility exists, the patient must be informed in advance and his/her consent must be obtained. Synthetic materials should be avoided whenever possible for repair purposes. It is possible that synthetic materials will not be accepted by the body over time and will be excreted and cause inflammation.

At the end of the surgery, tampons or different support materials can be placed in both noses, or it is also possible to fix them with stitches only, without placing any tampons. Suture application without tampons or tampons. The choice of surgery depends entirely on the habits or preferences of the doctor performing your surgery. However, no matter which method is used, normal and comfortable nasal breathing is not possible. Tampons are usually made of silicone and nylon-coated sponge and are removed within one to three days.

A plaster, metal or plastic splint (nasal mold) is placed on the outside of the nose. The purpose of this is to preserve the reconstructed shape and reduce swelling. Nasal splint and if there are stitches on the front, remove them after a week. ir. Sometimes, after the splint is removed, a patch is applied to the outside of the nose.

After waking up, the patient is taken to his room. It is recommended to sleep with your back high, talk less, avoid laughing, and keep your mouth open when sneezing. It is normal to have a slight bloody discharge from the front of the nose for a day or two. Intermittent ice application can be applied for a day to reduce swelling and bruising.

The patient is sent home the next day after staying in the hospital for one night. Antibiotics and painkillers are given when discharged. On the second day, they come to the hospital and their tampons are taken. Nasal tampons used today are very easy to remove and completely painless. There is a very significant relief after removing tampons. Later, the tissues inside the nose become blocked due to swelling, accumulation of discharge and crusting. It should be checked and cleaned from time to time. Nasal washing for the patient to clean himself at home; It is recommended to use oily drops, cream or spray for moisturizing. The patient can take a bath without getting his nose wet.

    HEALING PERIOD:

As in all surgeries, healing begins as soon as the last stitch is placed. Post-operative facial swelling and bruising under the eyes may occur. The severity of bruising may be more or less depending on the duration of surgery, the surgical technique, and the structural characteristics of the person. Most of the bruising and significant swelling disappear in ten days.

The recovery period of rhinoplasty surgery is very long and recovery is observed in three different areas:

  • Functional recovery: All effects of the surgery disappear and the person transitions to breathing with the desired quality within 30 days. – It takes up to 45 days. During this period, the patient takes care of himself at home, such as washing and oiling, and comes to the hospital once a week to have it cleaned.
  • Complete recovery may take up to one or two years.
  • Psychological recovery: Rhinoplasty surgery, its psychological aspect. It is a very important surgery. It will take a few months for the patient to get used to his new nose in the postoperative period, to stop examining his nose by constantly looking in the mirror, and to cope with various criticisms from those around him. In my opinion, real recovery is when the patient stops examining himself in the mirror.
  • WHO SHOULD PERFORM THE SURGERY?

Rhinoplasty surgeries take a long time. It is performed by amandır Plastic and Reconstructive Surgery specialists. In the last 20-30 years, it has been performed with increasing intensity by Ear, Nose and Throat & Head and Neck Surgery specialists. In fact, the most correct answer to this question is not which branch, but "whoever does it well should do it". Here, it is important that the physician's education, experience, aesthetic evaluation ability and ability to correct not only the shape of the nose but also its functions (such as good breathing) are important.
