Gallbladder Diseases

The purses we carry with us filled with money or gold have been a sign of wealth and prosperity since the Ottoman period. However, if the bladder we carry in our abdomen is full, it means the loss of our health, which is our greatest wealth, and the destruction of our well-being.

This organ, which is under the liver but attached to the liver, is 10 cm long and has a volume of 40-50 ml, is the gallbladder. The gallbladder, which plays a very important role in the ability of humans to digest what they eat and is the organ where bile is stored, is carried out by its collaborator, the liver, with an action such as storing roughly 1/3 of the bile, approximately 800-1000 ml per day, and flowing it into the duodenum at the appropriate stage of digestion. It helps digestion.

2/3 of the 1 liter of bile produced daily in the liver flows directly into the duodenum from the main canal. The remaining 1/3 is stored in the gallbladder, and due to the structure of the inner wall of the gallbladder, the liquid inside is absorbed and concentrated. and in this way its volume is reduced. When we eat, when the food digested in the stomach passes into the duodenum, the gallbladder contracts as a result of a hormonal mechanism and releases all the bile accumulated in it to the duodenum, helping the digestion of substances in the food, especially fats.

Gold does not mix with air. Bile, whose yolk turns dark green when mixed with air, is a very irritating substance. For this reason, when it fills the stomach, it irritates the lining of the stomach and causes a disease we call gastritis.

The most common disease of the gallbladder today is the stones formed in it. Gallbladder stones are basically divided into two groups: pigment and cholesterol stones. While pigment stones generally develop due to blood diseases, cirrhosis, inflamed bile, and obstructions in the bile ducts, cholesterol stones occur when the substance we call cholesterol crystallizes as a result of different effects and precipitates in the gallbladder.

70% of gallbladder stones occur. It should also be noted that 30% of cholesterol is pigment stones. Gallbladder stones, which we encounter 4 times more frequently in women than in men, are common in the public. For the first time in history, Pharaonic families in Egypt Although there is a group of gallbladder stones that do not cause any complaints and are called silent stones in medicine, 80% of these stones present with severe complaints and complications at some point in life.

What I have just tried to write about. Within the working system, the stone in the gallbladder blocks the exit of the gallbladder when the food reaches the duodenum, by contracting the gallbladder and moving the liquid inside towards the exit point, and thus the remaining bile cannot flow into the intestine, causing digestive disorders in the person. On the other hand, the gallbladder, whose outlet is blocked, cannot empty the fluid inside and continues to contract, thus symptomating this with pain. Since digestion is not completed due to insufficient bile flow, the person experiences complaints such as indigestion, rancidity and burning, as well as bloating as the amount of gas in the intestine increases.

Pains caused by stones in the gallbladder are caused by the consumption of foods such as oil and eggs, which play a major role in the digestion of bile. It increases greatly and sometimes these pains spread towards the shoulder and chest. Due to this feature of the pain, gallstones are sometimes confused with heart diseases. In fact, depending on the reflex mechanism between the gallbladder and the heart coronary vessels, it can cause a lot of pain in the gallbladder and even spasm in the coronary heart vessels.

In our daily lives, without realizing it, we use alternative medicine methods (soda, soda, etc.). It is an undeniable fact that we try to save the day by using some stomach and digestive medicines for our complaints (such as baking soda), often without consulting a doctor. However, if we know what the gallstones we manage like this can do to us, I am sure we can recognize and resolve this disease, which we can diagnose very easily with the conditions offered by civilization, without causing major problems.

Gallbladder stones are caused by foods, especially fat and cholesterol, in the digestive system. We mentioned that some complaints occur because rich foods cause problems during digestion. These complaints are complaints that disrupt the comfort of daily life. However The danger posed by gallstones is two other than these complaints, and both of them are caused by the obstructing feature of the stone. As we mentioned before, during each meal, the gallbladder contracts and discharges its contents into the intestine, and the stone in the bladder will move towards the exit along with the contraction. If the stone blocks the gallbladder outlet at this time, it will cause acute cholecystitis. If the stone passes this place and falls into the main canal, it will block the point where the main canal opens to the duodenum and cause obstructive jaundice.

Acute cholecystitis, severe pain, nausea, vomiting, fever, mild jaundice and pain in the back, shoulders and chest. It reveals itself with.

Obsessive jaundice is manifested by jaundice, nausea, loss of appetite and pain. In jaundice, the urine turns tea-coloured, the color of the stool becomes lighter and the entire skin, starting from the eyes, turns yellow. When jaundice is not treated, a process that leads to liver failure, which we call Biliary Cirrhosis, begins.

Apart from these two situations, pancreatic inflammation may occur due to stones blocking the lower end of the main duct. We do not often mention the fact that gallstones, which remain at the theoretical level because they have not been fully proven to date, cause gallbladder cancer. However, the fact that gallbladder cancers occur together with gallstones in 90% of cases is shown as the main basis of this theory.

An interesting situation regarding gallbladder stones is that these stones are called the 5F rule; It is more common in women who are blonde, over the age of 40, overweight, and who have given birth many times.

The only valid method for the treatment of gallbladder stones today is surgery. Surgically, as in our country, all over the world, the technique we call laparoscopic surgery, which people call closed surgery, is applied. Previously tried methods of breaking stones or dissolving stones with medication have lost their value because they did not yield the desired results and on the contrary caused some complications.

As a result of the risks imposed on the person by the previously applied open surgical method, silent gallstones, which constitute 80% of gallbladder stones, have lost their value. Stones that did not cause complaints could not be operated on until complaints occurred. Because in open surgery, there will be post-operative pain. The main reason for this idea was the patient's long-term loss of work capacity, apart from surgical site hernia and intra-abdominal adhesions.

Since closed gallbladder surgeries, which became widespread in the 1990s, minimized all these side effects, silent stones should also be operated upon when detected. has been recommended. The necessity of diagnosing and operating the patient without risky complications in order to perform closed surgery more easily has brought to the fore the recommendation of surgery as soon as gallbladder stones are detected in the new generation of thought.

This surgery, which is performed by magnifying the image 20 times, is developing rapidly and has become applicable to all abdominal surgeries.

There is a Turkish reality that I also accept. We do not even go to a doctor, let alone a surgery, for a disease that does not cause us any complaints. However, if we want to benefit from technology, which is the outcome of the information age, at the highest level, we have to give up these habits. In short, we must do our work before the egg comes to the door, as the people say. This idea will and should spread to the public under the leadership of us, who I believe are reading these lines and who have integrated information age technologies into their daily lives at the highest level.

Stay healthy, all of you.
