Non-Surgical Treatment Options for Peyronie's Disease

Peyronie's disease is a disease characterized by the formation of scar tissue called plaque due to penile tissue damage. Depending on the location and size of this plaque, the penis may bend in different directions and lose an erection. Although this disease is usually observed in middle-aged men, it can be seen at any age.

The following options are available in the non-surgical treatment of Peyronie's disease;

1-Drugs

In addition to drugs taken orally, some treatments can be applied directly to the plaque by injection, especially in the early stages of the disease. For example, there is evidence that injections of verapamil reduce plaque. Apart from that, interferon injections control the scarring, while collagenase injections break down and shrink the plaque. Apart from these, injections can be made using a combination of betamethasone, hyaluronidase or prostaglandin E1. While applying these drugs, iontophoresis (electromotor application / EMDA) application can increase the effectiveness of the treatment. With this application, the drug can be penetrated deeper by using a low electric current.

2- ESWT (Extracorporeal Shock Wave Therapy)

ESWT, with the help of a device It is the process of transmitting the produced shock waves to the area of the lesion by means of a probe and applying treatment. This treatment, which has been used in medicine for many years, was first used for the purpose of breaking kidney stones. Later, it found use in other medical fields and was used in diseases such as heel spur and tennis elbow. Shock waves are characterized by regeneration and new vascularization at the application site. In this regard, it is also used in the treatment of erectile dysfunction in men. It aims to increase the formation of new vessels, especially in erection problems caused by vascular occlusion. In this respect, it will stop the inflammatory process in Peyronie's disease and accelerate recovery. For this reason, the European Society of Urology reports that ESWT treatment can be applied especially in the initial and painful stages of Peyronie's disease. Although its effects on shrinking previously formed plaques are controversial, especially early return in the treatment of Peyronie's disease. is also recommended. Positive feedback has been received from the studies on its application in the treatment of Peyronie's disease, and therefore it has become one of the treatment options. In this respect, ESWT treatment is among the recommended options to be tried before surgical treatment.
