TOOTH SENSITIVITY and TREATMENT

What is Dentin Sensitivity and how does it occur?

The top layer of our teeth is the enamel layer, which has a hard and crystallized structure. Under the enamel, there are dentine tissue, which is a living tissue containing softer brick channels, channels we call tubules, odontoblast cells and dentinal lymph fluid. Intensely acidic foods and hard toothbrushes for the concern of cleaning and whitening the teeth, if the teeth are not brushed properly with harsh movements, there will be wear on the enamel over time. sensitivity occurs. Some women have a disease called Blumia, where they purge what they eat due to their obsession with gaining weight. In bulimia, the parotid glands become enlarged, and erosions, which we call Perimyolysis, occur, especially on the palatal back surfaces of the upper teeth, due to the constant exposure of the enamel and dentin to stomach acid. To prevent this erosion, the patient wants to clean his teeth harder and more frequently, and the result is tooth sensitivity. People with bruxism and grinding their teeth also experience sensitivity due to abrasions. If there are crown fractures on the tooth or teeth, undiagnosed interface caries, cracks that reach under the crown or gum of the tooth, which we call cracked tooth syndrome, this manifests itself with sensitivity and sharp pain. Carbamide peroxide is used to whiten teeth. If this substance is very concentrated and used for a long time, it may cause sensitivity in the pulp of the teeth due to hydrodynamic stimulation. Gum recession due to hard brushing, dehydration of the teeth during crown cutting, working with excessive pressure, and abrasives in some toothpastes can also cause dentin sensitivity. The rapid movement of the fluid in the tubules due to the stimuli that occur during the operative procedures affects the nerve endings in the areas close to the pulp and sensitivity occurs. Exposed dentin areas of the teeth that are exposed to erosion or abrasion cause routine plaque control not to be performed. Patients do not want to clean these areas because they hurt where the toothbrush touches, and as a result, they are exposed to gum problems and tooth loss.

TREATMENT

First, plaque control is taught. If sensitivity occurs due to abrasion, toothpastes containing potassium nitrate, soft toothbrushes and appropriate tooth brushing techniques are taught. If there is perimyolysis due to bulimia, a psychologist's help is requested, it is recommended to remove acidic foods from the diet, because even if you block the channels, acidic foods and drinks lead to redissolution and sensitivity, so eating habits should be changed. Resin lacquers, sealants, strontium fluoride toothpastes, sodium fluoride mouthwashes and dentin bonding are used in the treatment.
The most effective method for dentin sensitivity in the roots is sodium fluoride uIONTOPHORESIS. It precipitates and closes the dentin channels in a form of electrolysis with a very low battery current through the device. The treatment is very successful, it has no side effects because the fluoride used is in very small amounts, as a result, the sodium ion in sodium fluoride combines with the calcium in the teeth and the tubules are blocked in the form of calcium fluoride.
