The disease is caused by both bacteria called B hemolytic Streptococcus (colloquially beta microbe) and some viruses, and tonsils generally look like the one above. We, as physicians, make the differential diagnosis by performing a physical examination and start treatment accordingly.
➡B. Streptococcal infections begin suddenly, with a fever of 40 degrees Celsius that does not respond to antipyretics, sore throat, vomiting, difficulty in swallowing, and is sometimes accompanied by a rash. It is usually seen in people over the age of 3 and the only treatment is penicillin antibiotics. If left untreated, the fever can last up to two weeks. It may take longer. With treatment, the fever subsides in 2-4 days. Definitive diagnosis is made by throat culture. If some types of B Streptococcus bacteria are not treated, it can cause joint and heart rheumatism two weeks after tonsillopharyngitis, so these patients are re-evaluated after two weeks.
➡In viral cryptic tonsillopharyngitis, the fever is usually not very high and there are additional symptoms such as conjunctivitis, cold, diarrhea and hoarseness. It is common under the age of 3. There is no need to use antibiotics. Symptomatic treatment is given. The question that families ask and worry about is "Penicillin test is not performed." ? However, as a result of studies conducted all over the world, penicillin testing is no longer recommended. We recommend that injection treatment be performed in health institutions against the risk of allergy. Contrary to popular belief, penicillin allergy is not common.
