Holistic and Scientific Approach to Chronic Prostatitis

Chronic Prostatitis Disease seriously impairs the quality of life of men and can often put the patient in a state of helplessness. It is a collection of complaints whose exact cause cannot be fully explained.

First of all, in order to diagnose this disease, it is necessary to know the disease well. Unfortunately, since this disease is not well known, it may not be diagnosed for years. According to a study by Krigger et al., 2% to 16% of men under the age of 50 have this disease (1). Another problem is that since this disease is not fully recognized, the diagnosis period is on average 7 years (2). In my experience, diagnosis can only be made at least between 6 months and 2 years. The reason for this confusion is that the disease occurs differently in each person and affects each patient to different degrees. In some patients, persistent burning sensation during urination, testicular pain during ejaculation, frequent urination, leg pain, itching and a bad feeling inside the penis, pain and feeling of fullness behind the testicles and in front of the anus, inability to sit on a hard floor, the person constantly thinking about that area and feeling restless. It is a decrease or absence of erection, tingling in the head of the penis, meaningless aggression and irritability, and most importantly, a feeling of helplessness. Some patients are desperate enough to think about suicide and may even want to have their prostate removed surgically to get rid of these complaints.

It is really difficult to diagnose the disease, which manifests itself in such different ways. First of all, it is necessary to listen seriously and without interrupting the patient who is thought to have prostatitis. Because this patient has consulted at least 2 urologists before coming to you.

Possible causes of prostatitis were determined by Shoskes et al. from Cleveland Clinic, who conducted serious studies on this subject. (3) They explained in the World Journal of Urology as follows;

It can cause tissue damage in the prostate and bladder; These are infection, trauma, allergy or medication use, excessive stress and depression.

One or more of these may occur at the same time. According to a common belief, which in my experience is likely to be wrong, most causes are sexually transmitted infections. A disease causes this. But the most common type in the classification of prostatitis is; Nonbacterial prostatitis, that is, it is not caused by bacteria. Therefore, most of the time, no bacteria grow in urine samples taken from patients. It is a known fact that there is an inflammatory process in the prostate. A bacterium is not always necessary for an inflammation. For example, in case of extreme stress or trauma, the body's immune system activates chemokines and cytokines, causing inflammation in that area. Therefore, during prostate massage, the patient experiences pain because the prostate is inflamed, and during this time, when the fluid coming from the penis is examined under the microscope, leukocyte cells are seen.The number of 10 leukocytes or more is in favor of prostatitis. Again, clinical experience shows that whether prostatitis is present or not based solely on leukocytes is not a sufficient criterion to make a diagnosis. Of course, it is an important finding, and then taking a sample of that fluid and looking at 14 different sexually transmitted microbes with PCR will help us when planning the treatment. When the PCR result reveals bacteria, our job is relatively easier because we can prescribe antibiotics based on that result.

These 7 criteria should be considered when evaluating and diagnosing a prostatitis patient.

  • Is there a urination disorder? (Feeling of not being able to empty urine completely, relief after urinating, burning sensation while urinating)

  • Is there any psychological impact? (Feeling of helplessness, the idea that this disease has affected his whole life, or symptoms of depression)

  • Organ-specific (sensitivity in the prostate, blood in the semen, frequent urination)

  • Infection (prostate fluid culture is positive even though the urine culture is negative)

  • Neurological/systemic symptoms: Pain that may occur outside the pelvis

  • Sensitivity (Tenderness and pain in some muscle groups (Especially the perineal area - behind the testicles and front of the anus)

  • Sexual dysfunctions (Erection) problem, frigidity).

  • In the diagnosis phase, it should be checked whether there is a prostate stone by looking at anal ultrasound and, if necessary, the treatment plan should be taken into account. State massage should be added.

    After the diagnosis is made, the patient's condition should be explained in detail and the treatment phase should be started. While planning the treatment, 'Just as one size suit does not fit everyone, the treatment of this disease should be tailored to each person and adjusted accordingly. Because this disease manifests itself differently in everyone.

    Before starting treatment, it is necessary to KNOW THE DISEASE WELL.. Each patient must fill out the NIH-Chronic Prostatitis Symptom Index (NIH-CPSI) form and a treatment plan must be arranged according to the score obtained. For example, the treatment of a patient with urination disorder, muscle sensitivity, and bacterial growth or high amounts of leukocytes in the prostate fluid should be planned as prostate medication to correct the urination disorder or bladder medication called anticholinergic, kegel exercises to strengthen the pelvic muscles, and 6 weeks of antibiotic treatment. Antioxidant treatments used in the treatment of chronic prostatitis (quercetin, pumpkin seed oil, bee pollen, green apple peel, nettle) help treatment by reducing chemokines and cytokines that cause inflammation in the prostate. Vitamin supplements such as Liqone complex should be used for up to 3 months. Even if bacteria are not detected in the urine culture or prostate fluid culture, a weekly course of antibiotics should be recommended due to their anti-inflammatory properties.

    If the patient's psychology is very affected, getting psychological support is not a wrong approach and the recommended medications should be used for a while.

    Finally, if you understand and embrace the disease in every sense, there is no need to be afraid of it and you are not helpless. When there is a prostatitis attack, use appropriate multiple therapy (at least 2 different grams depending on the test result to be applied). You can easily cope with this disease with up to medication and vitamin support and experience attacks without disrupting your quality of life. Cold, being under stress, consumption of fermented alcohol, acidic drinks (orange juice, tangerine) exacerbate the disease, hot water sittings and apple cider vinegar can reduce your complaints.
