What is Varicocele?
It is a disease in which the veins of the testicle expand and twist, and the dirty blood circulation, normally directed towards the heart, returns towards the testicle.
Who Gets Varicocele?
Varicocele is a disease of young men. It is seen at all ages, starting from the age of 14. It usually occurs on the left side. It may be on the right or bilaterally.
What Kind of Complaints Does Varicocele Cause?
In addition to causing pain, it can be a nightmare for young men because it causes infertility. Its treatment is only surgery.
Should Every Case of Varicocele Be Treated with Surgery?
If it does not cause pain and does not have a negative impact on sperm count and characteristics, the person does not want or want children. If he has enough children, he does not have to undergo surgery. However, if there is a significant varicocele, that is, if swollen veins are visible in the left bag even when viewed from the front, if the egg on that side is shrinking, and if the sperm count is negatively affected, these cases must be operated on. There is also a group that is most abused, in which there is no clinically observable varicocele. However, varicocele has been reported on Doppler ultrasound. We are definitely not in favor of varicocele surgery for these patients with normal sperm count. If the sperm count is low, it is necessary to check whether there is reflux on Doppler ultrasound. If present, surgery may be recommended. Otherwise, their chances of benefiting from surgery are slim and these patients should be informed.
What Should Happen in a Man with a Normal Sperm Count?
In a Normal Man The number of sperm must be more than 20 million in 1 ml, at least 50-60% must be alive, at least 25% of the live sperm must move forward, the normal sperm rate must be between 4-14% according to the Kruger criteria.
How Should the Surgery Be Done in a Patient with Varicocele? Focus is on Microsurgical Method. What is it?
The type of varicocele surgery has changed throughout history. Ligation of the testicular vessels in the abdomen, high groin incision, laparoscopic interventions, radiological application of occlusive substances into the vessel. These are no longer applied.
Open classical surgery is now available everywhere. It is made by im. In this surgery, the veins visible to the naked eye are ligated, and smaller veins can be overlooked. This may cause the disease to recur in the future.
What is Microsurgical Varicocele Surgery?
In microsurgery, surgery is performed under a microscope with larger images. Even small vessels can be connected with approximately 8-16 times magnification.
Are there any other benefits of microsurgery?
Yes, there is. Thanks to the microsurgical method, the arteries feeding the testicle can be protected, thus preventing testicular atrophy or shrinkage that may occur in the future. Lymphatic white blood vessels are also seen and not connected. Otherwise, a complication called hydrocele, where water accumulates around the testicle, may occur. This means another unnecessary surgery in the future.
Is Microsurgery Varicocele Surgery Performed by Everyone?
It is performed by physicians with microsurgery certificates. There are benefits.
Is There Any Negative Aspect of Microsurgical Varicocele Surgery?
There are no negative aspects for our patients in microsurgical varicocele surgery.
11. Does Recurrent Varicocele Develop After Microsurgical Varicocele Surgery?
It develops at a negligible rate. Because in this surgery, all veins are connected under the microscope and arteries and white blood vessels can be fully protected.
12. When Do I Start My Current Activities After Surgery?
The next day you can go out and do your work. It is necessary to be careful not to lift weights for a week.
Is Varicocele Surgery Necessary?
Varicocele surgery is not always necessary. If the varicocele is mild, the patient has no pain, no anxiety, and the testicle has not shrunk, surgery is not necessary. There are some patients who do not have a varicocele on examination. However, varicocele has been reported with Doppler ultrasound. It is necessary to be more careful when deciding on surgery in these patients.
