Hydrocele (Water Hernia)

Hydrocele is the accumulation of excessive fluid in the sac around the testicle. Although it is mostly seen in newborn male babies, it also occurs in the following months. The cause of its formation is the same as that of an inguinal hernia. As the internal opening of the inguinal canal is too narrow to allow the passage of the organs in the abdomen, only intra-abdominal fluid passes through it and fills into the canal and bag.

There are three types of hydrocele, which are mostly seen in men. The first is a communicating hydrocele, the inner mouth of the inguinal canal has not yet closed, and the amount of hydrocele fluid changes from time to time. The second is non-communicating hydrocele. In this type, the amount of fluid generally does not change since the sac is not connected to the intra-abdominal area. The third type is cord hydrocele or cord cyst and fluid is trapped in the canal. Hydrocele, which is rare in girls, is called nuck duct cyst. It may also occur as a result of a condition that causes increased fluid in the abdomen after an infection in the testicles or vessels (kidney and liver disease, excessive fluid accumulated in the brain cavities - hydrocephalus - being drained into the abdominal cavity - shunt).

Communicated. Fluid in hydrocele may decrease or disappear after long periods of rest (e.g. in the morning hours), and it becomes more evident in the following hours as standing time increases. There is no change in the other two types. In intra-abdominal (communicated) hydrocele, the amount of fluid may decrease upon examination. For others, there is no change. Diagnosis is easily made during examination. Hydrocele is easily recognized by a test (transillumination) performed by shining a light on the sac in an environment with low light. There is no need to use any imaging method. It is distinguished from diseases that cause swelling in the bag.

As long as it does not reach excessively large sizes, it does not cause any distress to the baby. It may spontaneously regress and disappear completely in the following months. If it does not disappear, surgery is performed as soon as the baby's health allows, after 6-12 months, depending on the size of the hydrocele.
