Orthopedics and Traumatology Specialist Dr. Stating that 'rotator cuff' injuries, which is the most important muscle group that provides the movement of the shoulder joint, are one of the most common shoulder problems. Bertan Cengiz; “We meet our daily needs thanks to these muscles, which help us lift our arms, play sports, throw, push or pull an object. Life can become painful for us because of these tears, which are more common in people who have a serious load on their shoulders.” Providing information about rotator cuff injuries and tears, Dr. Bertan Cengiz stated that the gold standard in surgical treatment is the closed surgery method called 'arthroscopy'. Stating that although it is mostly seen in middle-aged people, this injury may occur earlier in people who do sports that put a serious load on the shoulders such as tennis, swimming and weightlifting. Cengiz, giving information about the causes of the tear; “A rotator cuff tear can occur in young people for acute reasons such as straining or falling during sports, usually while trying to push or pull an object. Repetitive strains related to work and sports are also an important reason. For example, in painters or tennis players, situations that require the arm to stay above the head for a long time facilitate the development of a tear. In advanced ages, we see the discomfort that turns into a tear, thinning and abrasion in the tendons. In addition, the decrease in blood supply of the muscles with increasing age or the presence of bone protrusions on the bone roof are other factors that facilitate tear formation.”
"Neck pain should not be confused with
Cengiz, who described the prominent feature of the rotator cuff tear as the severe pain that occurs especially when raising the arm above the head; He stated that the pain can sometimes be seen during rest and at night. Dr. Bertan Cengiz; "This pain, which starts from the shoulder and progresses to the upper side of the arm, can sometimes be confused with pain that develops due to neck problems. In addition, the person may experience loss of strength. The loss of strength in the muscles and joint stiffness that can develop over time are among the symptoms, especially when the patient does not use his arm to avoid pain." Cengiz said that the first step for the diagnosis of muscle tears around the neck is a detailed shoulder examination. "However, MRI has a special place in the diagnosis. It is the most frequently used and the most valuable diagnostic tool in the detection of both tears and other shoulder joint disorders. It is safely preferred because it does not cause radiation exposure," said Dr. Bertan Cengiz, in some cases to confirm the diagnosis. He says that local anesthetic injection is also applied into the gap between the torn muscle and the bone roof above the shoulder joint. It was also stated that treatment options may vary depending on the type of tear and the approach of the doctor. Stating that the first treatment is not surgery if muscle strength and shoulder range of motion are good in people over 65 years of age, in patients with partial muscle tears, Dr. Bertan Cengiz continued his words as follows;
"Treatment is started with anti-inflammatory drugs, appropriate exercises and physical therapy methods. Pain can be relieved with single-dose cortisone injections to the shoulder when deemed necessary by your doctor. In patients whose complaints continue despite these treatments, surgical treatment is performed. ”Closed surgery in the surgical treatment of rotator cuff injuries Emphasizing that arthroscopy, which is a yacht method, is considered the gold standard, Dr. Bertan Cengiz, his method briefly; “In this method, three or four 4-5 mm holes are drilled around the shoulder and entered with a special camera. Thanks to the holes drilled, the bone can be shaved, tears can be sutured, and if there is congestion or adhesions, they can be loosened," he summarized. Explaining that the images obtained in arthroscopy provide a great advantage compared to open surgery, Dr. Bertan Cengiz “Many places that cannot be seen with the naked eye in open surgery can be seen thanks to arthroscopy. Damage to healthy tissue that may occur during the operation is minimized. After the ruptured tendon is refreshed, it is fixed to the bone with suture anchors called "anchors". Stating that full thickness and wide muscle tears that cannot be repaired, treatment is provided by transplanting other muscles around the shoulder, Dr. Bertan Cengiz said, “Patients who develop damage to the shoulder joint as a result of being untreated for many years are treated by applying a shoulder prosthesis.”
