Pilonidal sinus (pilonidal sinus)

The disease, better known as ingrown hair, is a very common disease of the young population. The disease, which is common in the coccyx, can also be seen in different areas (armpit, between the fingers, belly button). Those formed in the coccyx are important because they are close to the anal region. Despite various treatment strategies, it is still a nightmare for patients because the disease recurs and affects the quality of life of patients.

It occurs when hair fills the space under the skin and the reaction fluid of the body accumulates. The resulting gap is opened to the skin through a mouth. These mouths may be more than one. The hairs that fall from the back and head get stuck in the hairy and sweaty groove between the two buttocks, which is the most hollow area of the back, and through friction, the screw-shaped hairs can penetrate under the skin. The resulting cavities and labyrinths are suitable areas for inflammation. Abscesses form due to bacteria entering here.

Clear dark discharge, may be bloody. With inflammation, a reddish sensitive area may occur in the coccyx. If the infection is severe, it can cause fever, fatigue and nausea. Rarely, it can spread to the bone on which it is located and cause serious inflammation that can be life-threatening.

1) Non-surgical applications:

In recent years, crystallized phenol applications have become current as a non-surgical treatment method. We have received extremely successful results from our follow-up patients. It is applied under office conditions and the patient returns to his normal life after an hour. In this regard, silver nitrate applications and laser applications have also become current in recent years.

2) Surgical applications;

a) Procedures with flaps: They are more advanced and complicated procedures and have lower recurrence rates.
b) Procedures without flaps: These are procedures with higher recurrence rates. The operations to be performed should be discussed with the patient and a decision should be made after informing them.

In the treatment of pilonidal sinus, the area between the hips should be kept clean from the moment the patient is first seen, if possible. It should be cleaned with olive oil soap, and the patient should be reminded that cleaning the hair in this area is a priority. It is necessary to continue paying attention to this area after the treatment.
