Smoking's effect on the tissues in the mouth is to prepare the ground for bacterial plaque retention, increase the risk of infection, delay wound healing and reduce blood flow.

The prevalence of smoking in our country places the responsibility on us, as dentists, to be informed about the effects of smoking on oral tissues. Especially in periodontal surgical procedures where patient expectations are high, the consequences of smoking should be well known and patients should be warned about this.

The aim of periodontal treatment is not only to stop the progressive periodontal disease, but also to ensure the return and regeneration of the destroyed periodontal tissues to their old healthy state.

Many methods have been applied to date for the treatment of bone defects that occur as a result of periodontal disease. We can examine these treatment methods in two main groups as resective and reconstructive methods:

Smoking, in addition to its negative systemic effects on human health, also negatively affects the oral tissues with the toxic substances it contains and the heat released during its use. The heat released causes an increase in keratinized tissue in the oral mucosa. The nicotine and other toxic substances it contains may cause deterioration of wound healing after periodontal surgery with their systemic and local effects. Nicotine reduces the proliferation of defense cells, per Causing constriction in peripheral blood vessels and disrupting the functions of connective tissue cells are the main reasons for this negative effect.

Impairment of wound healing may cause deterioration of patient comfort after the surgical procedure, failure to obtain the desired result from the treatment, and failure to meet patient expectations. In some cases, this problem may require a second surgical procedure, and this causes various losses for both the physician and the patient. In order to avoid such consequences, the dentist must first know very well the harms that his patient's smoking will cause after the periodontal surgery procedure and inform the patient before the procedure. If cooperation with the patient cannot be achieved, the benefit and harm ratio of the procedure can be evaluated and the application can be abandoned.
