Breast Aesthetics (Mammoplasty)

In addition to its functional importance during breastfeeding, the breast is also an extremely important organ visually for the female body. In fact, in many cultures today, the female organ is seen as breast. For this reason, breast size being less than desired (breast hypoplasia) is a condition that we frequently encounter to be corrected.

Breast hypoplasia is a condition that occurs both during developmental youth and repeated breastfeeding processes over the years. It is frequently detected in later ages due to reasons such as hormonal changes. In this case, we, plastic surgeons, apply surgical techniques that increase breast volume. Although there are different techniques used for breast enlargement today, the most commonly used one in the world is breast enlargement with silicone breast prostheses. The second technique is fat injection, which has been preferred in recent years, especially in cases where more limited augmentation is required or asymmetric correction is required. We perform enlargement with silicone breast prostheses in the majority of patients who apply to us with a request for breast enlargement.

Breast enlargement with silicone breast prostheses is a procedure performed under general anesthesia and takes approximately 1.5 hours. is the process. The breast prosthesis is basically placed in two planes: Under the muscle (Under the pectoral muscle = Submuscular) and Under the mammary gland (Subglandular). This decision is made based on the condition of the patient's breast tissue and chest muscle (pectoral muscle). The surgery is generally performed using one of two different incisions: in the lower half of the colored area (areolar) around the nipple (circumareolar) and in the submammary fold area. The incision is approximately 4 cm and when planned correctly, it heals imperceptibly. Before the surgery, during the interview with the patient, the breast size that the patient wants to achieve after the surgery is determined based on both the physician's guiding questions and the available images. During the surgery, after the space in which the prosthesis will be placed is created, the appropriate volume prosthesis is determined and placed after washing with antibiotic solutions under sterile conditions. After the surgery is completed, the patient is dressed in a special bra that she will use for 4 weeks after the surgery and is taken to the patient bed.

Breast size In augmentation surgery, an important factor affecting the postoperative period is the plan of the prosthesis. Prostheses placed under the pectoral muscle cause tension in the muscle and cause more pain than prostheses placed under the mammary gland. This pain is most intense within 12 hours after the surgery, and therefore the patient is kept in the hospital on the night of the surgery and appropriate painkillers and muscle relaxants are administered. The first check-up is done 7-10 days after the surgery. In this control, stitch lines are checked. The final result of breast augmentation surgery occurs more quickly (approximately in the first month after the surgery) than in many other plastic surgeries. In the month after the surgery, it is sufficient to use a special bra and avoid activities that require strong use of shoulder and arm muscles.

Breast reduction

Breast tissue is of great importance for women's body image with its shape and size, as well as breastfeeding function. In the breasts; Volume increase and sagging may occur due to familial predisposition and developmental reasons, or due to breastfeeding and weight gain. This deformation; It can cause shoulder and back pain, redness under the breasts, diaper rash and ulceration, and even neck-lumbar hernias. Considering the increase in breast cancer in recent years, it should be taken into consideration in all breast surgeries, even if they are aesthetic, and all patients over the age of 40 should be screened with breast ultrasound or mammography before surgery.

Breast reduction. The surgery is performed under general anesthesia and takes approximately 3 hours. Basically, two types of incisions are used in the surgery, depending on the size of the breast tissue. The first of these is a vertical incision (a vertical scar extending around the nipple and below), and the other is a Reverse-T incision (an additional scar along the sub-breast fold in addition to the vertical incision). While an inverted-T incision is required for sagging and larger breasts, a vertical incision is used in cases that require lift rather than reduction. Liposuction is also applied in addition to patients with excess fat, especially in the armpit and outer side of the breast. At the end of the surgery, the patient is put on a bra specially produced for the surgery, taken to bed, and spends the night in the hospital.

Patients are discharged the day after surgery or the day after that (when the amount coming from the drains decreases). They start showering within 48-72 hours after the surgery and usually return to work within a week at the latest. The first check-up after the surgery is done in the first week and the suture lines are checked. The special bra worn at the end of the surgery is used for an average of 6-8 weeks, after which patients switch to the bra of their choice. The final result of the surgery becomes clear around the 3rd month, when edema disappears to a high extent. However, patients feel the lightness and comfort of getting rid of the weight from the first hour after the surgery.
