The type of birth that pregnant women fear the most is trying a normal birth using artificial pain.
The basis of women's fear of this is that they have a wrong or incomplete knowledge of the event. They lose the chance to give birth normally because of the thought 'I would have a cesarean section rather than artificial pain.' I would like to give brief information about this subject so that pregnant women can make their decisions more easily.
Initiating labor with a drug called artificial pain is labor induction. Uterine contractions, which are necessary for normal childbirth, begin to occur. Triggering birth is done if there is a problem with the health of the mother or baby or if the pregnant woman's 42 weeks of pregnancy are completed. In some countries, women give birth at the planned time using artificial pain at 39 weeks. This can be called the application of arbitrary artificial pain
. This is not a practice I approve of. Spontaneous onset of pain always increases the chance of normal birth.
In order for birth to occur, the cervix must be soft, thin and open. In fact, these changes
begin a week or two before labor begins. Giving artificial pain is effective if the cervix is soft and ready to open
. Whether it will be effective or not can be determined according to the Bishop's evaluation by the doctor.
If there are no such changes in the cervix of the pregnant woman for whom artificial labor is planned, medications that will soften the cervix are applied vaginally one or two days before. Artificial pain is usually done by administering oxytocin, the contraction hormone, intravenously. This drug used to be given as an injection into the hip in high doses, and
the sudden excess of contraction hormone in the body caused a lot of pain as it contracted the uterus violently.
Now the oxytocin drug is given almost at a physiological dose. Therefore, it imitates normal contractions.
By evaluating the condition of the contractions, the most effective dose is adjusted, starting from the lowest dose and increasing it every 15 minutes.
Pills inserted vaginally also cause contractions. and is applied every 4 hours. Side effects of these drugs
: Phase contraction of the uterus and temporary decrease in blood flow to the baby, uterine rupture, emergency
caesarean section. Harm or side effect of artificial pain during cesarean section It is much less than normal.
If contractions do not start after the medication is given, the woman can go home and try again a day or two later. If birth does not occur after the 1st or 2nd artificial pain attempt, a cesarean section is performed.
