DIAGNOSIS AND TREATMENT OF NOSE BREATHING PROBLEMS

The main function of the nose is to ensure healthy breathing. is to provide. You are considering rhinoplasty; but you also have breathing problems. So how are these breathing problems diagnosed and treated?

First of all, the first stage is the examination of the inside of the nose with the help of a tool called a speculum. In this way, an area of 1/3-1/2 of the nose can be seen clearly. Septum (cartilage and/or bone) curvatures, flesh (turbinate, polyp) size or nasal canal stenosis and nasal stones can be seen in this area. However, in order to evaluate the more posterior parts, illuminated camera systems (endoscope), which we call rhinopharyngoscope, are needed. In this way, we can perform endoscopic examination of our patients who apply to our clinic with breathing complaints. In addition, we can clearly detect the location of your curvatures, flesh sizes and even stenosis in the nose with our acoustic rhinometry device, which works numerically with sound waves. Thanks to these two devices, there is less need for tomography imaging methods, which are another way to see the back parts of the nose but include X-ray.

We have imaged the disorders in your nose that cause breathing problems, so what? How effective and high quality are you breathing? If Apart from the marrow, deficiencies in your nasal cartilages can also make it difficult for you to breathe. For the diagnosis and post-treatment follow-up of such conditions, we use two systems called rhinomanometry and PNIF. These show us the amount, shape and quality of your breathing.

After all these processes are completed, we have clear and scientific information about your breathing. In this way, it is possible to solve your breathing problems simultaneously while having your rhinoplasty surgery in the light of scientific techniques.

CAN MY BREATHING PROBLEM CAN BE FIXED WITH MY NOSE AESTHETICS SURGERY?

During aesthetic nose surgery, any respiratory problems related to the nose (such as deviation, valve insufficiency, flesh size), if any, are treated simultaneously. It is best to have these two surgeries simultaneously. Because the parts removed during respiratory surgery can be used in the plastic surgery section. If you have breathing surgery before, you will not have such a chance.

WHAT WILL BE THE RESULT OF MY NOSE AESTHETICS SURGERY?

Regarding your rhinoplasty surgery This is perhaps the subject you are most curious about. For this, we take standardized pictures from various angles. Then, we scan your face in 3D with our Vectra 3D device. The system automatically provides us with the length, width, curvature of your nose, its compatibility with your face, various angles and proportions and many other parameters. After making these calculations, we perform your surgery simulation with you using your 3D images. In this way, when you leave our clinic, you will see your surgery result in 3D, as close to reality as possible.

SURGERY IN RHINOplasty TECHNIQUES

Dr. Kamburoğlu performs rhinoplasty surgeries under general anesthesia. Because it is difficult to ensure full patient comfort under local anesthesia. However, under general anesthesia, blood pressure can be kept at the desired level; Bleeding can be minimized. On the other hand, smaller procedures can be performed with local anesthesia rather than a full nose surgery.

Aesthetic nose surgery can be performed in 4 different ways. The healing and recovery processes of all these methods differ from each other.

1) Open Technique: During this surgery, the surgeon makes a small transverse incision in the columella (the column that separates the two nostrils) and It peels the skin from the tip of the nose and delicately shapes the bone and cartilage inside. This procedure leaves a small scar on the lower part of the nose, which is often not noticed at all. The advantage of this technique is that it provides full control of the nose and allows the tip of the nose to be firmly shaped with stitches. The disadvantages include a slight scar that may remain in the columella and swelling that lasts a little longer than the closed technique. Although the mentioned disadvantages are very rare, this is the most commonly used nose surgery technique in the world today.

2) Closed Technique: In this technique, since the incisions are made inside the nostrils, they are almost There is no visible scar left. On the other hand, in classical closed techniques, the tip of the nose cannot be shaped with stitches, as it is not possible to control the tip of the nose as in the open technique. This technique, which can be preferred in patients who do not need to shape the tip of the nose, has lost its popularity today.

3) Delivery Method: In this technique, nasal cartilages are delivered through two parallel incisions inside the nose. It is formed and shaped. The advantages of this technique are that it provides dominance as in the open technique, and that no incision is made in the columella (the column that separates the two nostrils) unlike the closed technique. The disadvantage of this technique is that one of the parallel incisions must be made in one of the nose tip support mechanisms (scroll area). In this technique, there is a widespread consensus that the scroll area should be repaired at the end of the surgery in order to prevent this structure from causing drooping nasal tip.

4) Modified Delivery Method: This technique is used to prevent nasal tip ptosis. allows the tip to be visualized as in the open technique; It does not leave a scar on the columella (the column separating the two nostrils) as in the closed technique. Additionally, since a second incision is not made in the nose as in the birthing technique, the nasal tip support mechanisms are preserved. Dr. This method, developed by Kamburoğlu by adding new suturing techniques, was published in the Aesthetic Plastic Surgery magazine, the official publication of the World Association of Aesthetic Surgery (ISAPS), in 2014.

Dr. Kamburoğlu generally prefers the 4th ranked modified delivery method and the 1st open technique in his surgeries. Although the closed (modified delivery) method is the appropriate technique for many primary patients (who have not had surgery before), in some cases (such as the presence of previous surgery, curvature situations, very small nostrils, etc.), the open technique may also be preferred.

ULTRASONIC RHINOPLASTY (ULTRAPLASTY)

One of the most important parts of rhinoplasty surgery is undoubtedly the bone shaping process. This procedure should be designed and performed in full harmony with both your forehead and mid-face, as well as your chin. In classical rhinoplasty, this procedure is It is performed with surgical instruments and micromotors. In ultrasonic rhinoplasty, this procedure is performed very precisely with the help of an ultrasonic surgical system, without causing any damage to soft tissues and cartilages. This technique provides a significant advantage over classical methods, especially in patients with crooked noses or thick bones. The ultraplasty systems we use consist of device-tip combinations specially designed for rhinoplasty. All kinds of techniques, tools and devices that you may need during your rhinoplasty process are at your service in our clinic.

NON-SURGICAL METHODS IN NOSE AESTHETICS

These techniques are used to transform the nose. It is based on the principle of shaping with various fillers. Although they seem quite easy, they are rarely used among plastic surgeons because the results are not permanent and lumps may occur, especially in the bone areas.

NOSE AESTHETICS TYPES

Rhinoplasty surgeries; It can be divided into 3 groups: reduction surgeries, enlargement surgeries and surgeries where both reduction and enlargement techniques are applied. Only in patients who have undergone reduction, the tissues removed are used to reshape the nose. In patients who need augmentation, ear cartilage, rib cartilage, abdominal muscle membrane or membranes taken from under the scalp without damaging the hair and skin can be used. Augmentation surgeries are mostly needed in the treatment of secondary patients (who have previously had surgery in another center).

OPERATION DURATION

Operation duration Although it varies depending on the procedure to be performed and the technique to be applied, it is approximately 3 hours on average. The definitive end point of all surgeries is that your surgical outcome will be determined by Dr. Kamburoğlu was comfortable with it and said ' It's the moment when you say 'it was so nice...' The more you care about a structure that stands in the middle of your face and will play a role in your happiness throughout your life, the more we care.
