We call female-type breast enlargement in men gynecomastia. It is natural for there to be fullness and sensitivity in the breasts due to the change in hormonal balance during adolescence. However, this situation, which should be temporary, may not subside for various reasons. This condition, which mostly occurs spontaneously due to structural reasons such as excessive weight gain, may rarely indicate endocrinological and urological problems. A detailed history, imaging techniques and hormone analyzes may be required for differential diagnosis. It may occur as an enlargement of breast tissue, excessive fat accumulation in the breast, or a combination of both. Gynecomastia occurs in 3 degrees. In mild gynecomastia, there is a slight protrusion of the breasts from the body. Medium breast size is divided into two subgroups depending on whether there is excess skin or not. In advanced gynecomastia, the breasts appear in female form. Both excessive size of the breasts and sagging of the skin are observed. Treatment is often surgical. The surgical technique is decided according to the degree of gynecomastia. After the examinations, there is an increase in the fat around the breast tissue, but if there is no abnormal increase in the breast tissue and skin, it is possible to enter the area such as the armpit or nipple with a small incision of 1 cm and aspirate the fat tissue in the breast with the help of thin cannulas. If this condition is accompanied by an increase in breast tissue, then the breast tissue should be reached through an incision made around the nipple and the breast tissue should also be removed. Since the incision will be in the transition area between the brown area of the breast and the skin, the scar will not be noticeable. In cases where severe skin sagging is accompanied, open surgery is considered. In this case, the breast tissue and surrounding fat tissue are removed, and excess skin is also removed and shaped. In this case, depending on the technique used, a scar will remain in the form of a thin line on the under-breast fold line, around the nipple, and in some cases from the nipple to the under-breast fold line. This scar fades over time.
