One result of the damage caused by gingival inflammatory diseases (Periodontitis) over the years is the formation of displacements and gaps in the teeth due to the disruption of the balance that keeps the teeth in their proper position. This situation is called (MIGRATION) - migration, relocation. It is often seen in one or more front teeth. Movements in the teeth also accompany this situation, and sometimes displacements occur along with elongation and sagging. This is also called (DRIFTING). It is the destruction of the ligaments connecting the teeth to the jawbone and the movement of the teeth as a result of the destruction of the tissues caused by decreased bone support and chronic inflammation.
Patients mostly complain that their teeth are protruding forward or out of the correct arch line, tipping over, and gaps are formed. They come to us complaining about aesthetic defects and fear of losing their teeth. The upper incisors are drifting and leaning forward. Sometimes, teeth protrude into the mouth and their root surfaces become exposed. This situation is defined as (EXTRUSSION).
The maintenance of the normal position of the teeth is ensured by 3 basic factors.
1- Healthy gums and surrounding tissues (PERIODONTIUM)
>2- Pressures of the tongue, cheek and lip muscles
3- Chewing pressures on the teeth
Pathological Migration occurs as a result of the weakening of the support of the teeth and the increase in the load on the teeth. . p>
HOW IS THE TREATMENT? .
First of all, the cause is investigated and the treatment is given accordingly. Periodontal Inflammations (PERIODONTITIS) are eliminated with appropriate surgical interventions and recovery is evaluated. After Periodontal Surgery, the tooth or teeth that have changed location may be strengthened and returned to their previous position, and the movements in the teeth may decrease or disappear. I can state here that in many of my patients whom I have treated by performing Flap Operations for more than 40 years, I have observed that the gaps in the teeth have mostly disappeared and the play has decreased or disappeared. The aesthetic problems of the patients are solved after the Flap operation by eliminating early contact points and by balancing the pressure. It is solved by connecting the teeth to each other with a method called splint, or by using aesthetic block bridges, both by correcting the aesthetic defects in the teeth and by connecting them to each other, preserving them in a functional state for many years. The physician and the patient should discuss alternative treatment methods and decide together on this issue.
