Anal Fissure (Anal Crack)

It can be seen at any age. It usually starts after constipation. In women, pregnancy and difficult birth may also cause it. Severe pain, bleeding and palpable swelling in the anus may be present. The diagnosis is made by examination. In suspicious cases or in the presence of other intestinal complaints, endoscopy may be required.

Medication is the first option if the pain is not unbearable. If the disease does not go away after drug treatment, other methods are used. In case of pain or serious bleeding, direct surgical methods or botox can be tried.

Classical operation technique: It is performed by numbing the waist or with general anesthesia. It is a short surgery. A part of the hypertwitching muscle of the anus is cut through a small opening 1 cm to the side of the anus. It may sometimes be necessary to remove the crack. After surgery, patients are usually kept in the hospital for 1 night. The healing time of the postoperative wound is 2-3 weeks. Dressings and wound hygiene are important in this process.

Laser surgery: What is done in classical surgery is done with laser. However, a much smaller opening is made for the laser, and if it is not necessary to remove the crack, the process is completed. General anesthesia is generally preferred. There is no need for dressing. It still takes 2-3 weeks for the crack to heal.

Botox application: It paralyzes the muscle that was cut and loosened during surgery for a period of 3 months, allowing it to relax, in other words, it mimics the surgery. It is performed by injection from the side of the anus. It does not require anesthesia. It does not require hospitalization after the injection. After botox, drug treatment is required for a while.

There is a risk of recurrence after all interventions. In such a case, action is taken according to the condition of the newly developed crack. Inability to defecate after surgery There is a possibility, although low, but this probability increases with age and the presence of other diseases in the patient. This side effect after botox is temporary.
