There is a lot of information written about the causes of acne and many methods used in its treatment. Finding and solving the root cause of acne is the main thing in the treatment approach. The cause of acne, which is enough to make all of our lives unbearable, and sometimes does not stop progressing no matter what we do, and spreads from the face to the neck, back and chest, is not only the problems on the surface of our skin.
The most common acne is typically in the T-zone or chin area of your face. Acne appears as a mixture of whiteheads, blackheads, and general redness around the affected area. This type of acne is called 'Acne vulgaris'.
Another type of acne is called cystic acne, which forms under the skin like a hill and never shrinks. This type of acne tends to go deeper into the skin. They tend to be painful to the touch and more persistent. Cystic acne is mostly found in the lower half of the skin and on the chest, back and shoulders.
Having acne is, of course, mentally disturbing. It has been discovered that people with chronic acne are more prone to depression. People with acne experience discomfort in social environments. They are judged as if they are considered unclean by the social environment. Yes, although skin cleansing is very important in acne formation, it is not that simple. Even if people with acne eat healthier and wash their skin frequently, these two measures may not solve acne.
Acne is observed in more than 50% of people during adolescence. Although most people's acne ends in their late 20s and early 30s, it tends to persist stubbornly in some. Adult acne tends to be cystic and occur on the lower face and neck, décolleté and back areas. During adolescence, we can at least predict when acne will regress. The biggest problem with acne in adulthood is not knowing when it will regress. This makes people feel like they are in a tunnel with no light at the end.
The first thing we need to do to solve acne is to find the underlying cause.
It is known that acne, especially in adolescents, is caused by hormonal imbalances. Especially if you are older, menstrual It is known that the cause of acne formation in women during the rumination cycle is hormonal. Well, it is hormonal, but why does an imbalance occur in hormones? The most important reason for this, especially in adults, is; It is estrogen dominance.
Some of the Causes of Hormonal Acne;
1.Estrogen dominance; It describes an increase in estrogen levels versus progesterone levels. Normally, estrogen and progesterone are in a balance; when the balance is disrupted, the increase in estrogen versus progesterone is called estrogen dominance. This may also cause conditions such as appetite imbalance, mood change, headaches, anxiety, and is also one of the most important causes of acne. 2.Androgen excess; Testosterone causes excessive production of Sebum and keratin on the skin surface. This causes the pores to become blocked. Among the causes of excess androgen; polycystic ovary syndrome, stress-induced disruption of the adrenal axis. Other symptoms of testosterone excess include; hair growth, irregular menstrual periods, apple-shaped weight gain.
3. A familial hormonal problem, that is, genetic transmission, may play a role in hormonal acne. If there is a condition such as PCOS, early hair loss, prostate hyperplasia in the family, genetic predisposition should be taken into consideration.
After hormonal reasons, I would like to draw attention to nutrition.
Regarding nutrition. Factors that can cause acne include foods that we know are inflammatory. These foods are;
Gluten, milk and dairy products, and refined sugar, which we definitely cut out in the elimination diet. These cause inflammation, that is, a toxic reaction, to begin in the intestine and subsequently throughout the body, and excessive increase in inflammation causes damage to various organs and systems in our body. One of these consequences is acne.
Other foods that we can count among inflammatory foods and that we know that frequent consumption causes acne include foods with a high glycemic index. Foods with a high glycemic index cause excessive secretion of insulin and another substance we call IGF1. The receptors of this substance are also found in the sebaceous glands responsible for the formation of acne on the skin. Therefore, its excessive release causes acne formation.
Acne Another reason for its formation is the insufficiency of detoxification pathways.
Our body has complex detoxification pathways consisting of the liver, gallbladder, kidneys and urinary tract, intestines, lungs, lymphatic system and skin. Each of these organs plays a critical role in keeping the level of toxins and waste in the body balanced. Liver detox systems are especially important in acne formation. Supporting liver detox systems in our patients suffering from chronic acne is one of our most important approaches.
And as the reason that plays the most role in all of our lives; Stress. As a result of stress, the body secretes cortisone hormone from the adrenals. Since cortisone, sex hormones such as estrogen and testosterone are synthesized through the same pathways, hormonal balance is disrupted. Stress affects the adrenals in this way, causing hormone imbalance in your body and may cause acne.
Another issue we have been focusing on recently is the connection between the intestine, the immune system and the skin. We can explain it briefly as follows: An imbalance in the intestinal flora can activate the immune system due to bacterial endotoxins and lipopolysaccharides. Increased inflammation as a result of excessive stimulation of the immune system cells in our skin causes the skin barrier to deteriorate and subsequently bacteria easily settle and cause acne. Another part of why intestinal dysbiosis causes acne is SIBO. Impaired motor movements in acne cause constipation, which further increases inflammation and causes acne.
It is very important to elucidate the underlying mechanism in order to treat acne and get rid of it forever.
