Bichat Fat Pad

1. Bichat fat pad is a deep fatty tissue system that starts from the temple area and extends to the cheek and supports the recessed areas of the skull skeleton. The cheek extension of the Bichat fat system may be too prominent and herniated in some people. This herniation may make the inner half of the aesthetic groove drawn between the earlobe and the corner of the mouth, which we want to be relatively empty, appear fuller than desired. In this case, the cheek fat pad can be removed or shifted to another position.

2. Most of the patients who request this procedure are in the young age group. Bichat fat pad does not have as significant an effect on facial contour in healthy young individuals as the patient thinks. Since it is a “deep” fat system as its name suggests, the tissue layers on it often mask the visibility of this structure from the outside. Our young patients who request this procedure literally desire a change in the model of their faces. You don't have the chance to change to a new face model today with facial aesthetics and say "I'm bored with it" tomorrow. Therefore, you should be sure whether the cheek shape you see and like on the magazine cover can be achieved on your face, and even if it can be achieved, whether it will suit you or not, and whether you really want such a change.

3. Patients think that when this fat pad is removed, their face will suddenly become thinner and take a V shape. However, face/cheek width; It is affected by many different factors, from the angle of the jaw bone to the position of the tip of the jaw, the volume of the chewing muscle, the size of the cheek salivary glands, the individual anatomy of the subcutaneous fat layers, the genetic shape of the face and aging characteristics. You cannot make the face thinner when viewed from the front by simply removing cheek fat.

4. The Bichat deep fat system does not widen the face to the side, rather it pushes it forward. There is a basic principle that surgeons dealing with facial aesthetics know; As the skeletal surface of the face moves forward, the face appears thinner when viewed from the front. As the skeletal surface of the face moves backward, the face appears wider when viewed from the front. In other words, reducing a large amount of deep fat on the cheek can make the face look wider, the opposite of what is desired.


5. The cheek extension of the Bichat deep fat system fills the groove of the maxillary bone, which we call the canine fossa. This groove already wears out and becomes more hollow during the aging process. When we reduce the fat in this area, the nasolabial fold between the cheek and lip can suddenly become deeper. Some patients express this situation as if they look older after the procedure.

6. During the aging process, your face will lose so much volume that you will search for every gram of fat. In the treatment of the aging face, we may even need to enlarge the deep compartment containing this fat with external adipose tissue supplementation. I use this oil to achieve a higher level of volume in patients who have deep plane facelifts. We use this oil to fill the groove of the skeleton between the cheek and nose in patients where we perform mid-face lift. Therefore, the use of this fat in youth eliminates the possibility of surgical use in later years.

7. A significant part of the surgeons who remove the Bichat fat pad remove the cheek fat they remove to protect their patients from the effect of "net volume loss". or they process the fat tissue they take from another part of the body and inject it into higher parts of the face. Remember, anything that creates a net volume loss on the face (iposuction, lipolysis, focused ultrasound that destroys subcutaneous fat mass, botox, bichectomy, etc.) will cause signs of aging to appear earlier in the long term.

8 .Removal of the cheek fat pad is a surgical procedure. It is performed through an incision inside the mouth. As with every surgical procedure, it has risks, complications and a unique recovery process. Be sure to ask these questions and learn.
