There is a sac called bursa above the tendons (the fibrous connective tissue that connects the muscles to the bones) in the shoulder. This vesicle shoulder may swell or have bony prominences due to overuse. This picture is called shoulder impingement.
What are the Symptoms of Shoulder Impingement?
As soon as the patient lifts his shoulder, he feels pain and becomes restless. Although the pain is not very severe at first, he goes to the doctor because his movements are restricted.
What are the Causes of Shoulder Impingement?
It occurs due to overuse of the shoulder. Repetitive movements over the shoulder are the biggest cause. We see shoulder patients very often due to reasons such as doing sports and using the arms a lot in daily life. Sometimes it may occur due to reasons such as tears and calcifications.
How to Treat Shoulder Impingement?
If there is no problem such as tear or calcification under shoulder compression, by strengthening the muscle groups with physical therapy, it can reduce the edema of that area. The problem is resolved by injecting cortisone into the bursa. Especially cortisone injections are very useful. Patients are very afraid of cortisone, saying, 'Oh, I will gain weight, I will eat without salt'. However, these are locally effective cortisones, that is, they do not cause side effects. In addition to relieving symptoms, cortisone is also therapeutic. It treats edema by eliminating it.
Is Surgery Necessary for Shoulder Impingement?
Sometimes there are such chronic cases that it is not possible to relieve tissue edema. Physical therapy is not enough. It gives the patient such a problem that especially night sleep is very painful. Then a surgical intervention comes into play.
What Causes Tendon Tear?
The place where the muscles that move the shoulder in the shoulder joint stick to the bone is called a tendon. For example, if you do sports such as swimming, volleyball, basketball, and handball, if you use your arm on the head very often, if you work with your arm open to the side in your profession, you are at risk for tendon rupture. Tendons are between two bones, due to repetitive arm movements, they get stuck between those two bones over time, first an edema occurs in that area, then the structure of the tendon begins to deteriorate. After that, partial tear becomes the last total tear. Tendon tears also occur in sudden falls. The tendon may rupture during a fall on the arm. spinner case for this called a tear. The treatment for this is surgery, except for very old patients. We used to operate on tendon tears by opening them. It was painful for the patient, and post-operative physical therapy was very difficult. But now almost all operations of the shoulder can be performed with arthroscopic (closed) surgery.
