Cancer and Laparoscopy

Unfortunately, stomach and large intestine cancers are among the important problems of our age.

Today, surgical treatments, that is, surgeries, still constitute the backbone of the treatment of these diseases. Except for some patients who are late, the main step in the fight against this disease is in the operating room. Preoperative medication and radiation treatments are used in a selected group of patients, and patients are operated on after a certain period of time after these treatments.

Are laparoscopic surgeries suitable for every patient?

If there are no obstacles to surgery in terms of anesthesia or surgical technique, surgeries of all stomach and colon cancer patients can be performed closed. However, generally speaking, it may not be suitable for some COPD patients with severe heart failure, which may negatively affect airway pressure. This is a situation that should be decided by discussing with our doctor friends in the anesthesia team.

Beyond this, laparoscopy may be insufficient when removing tumors that have grown excessively or are deeply ingrained in the vascular structures. While this situation can sometimes be predicted with preoperative examinations, sometimes we may need to start the surgery closed and decide that laparoscopy is not suitable during the surgery and continue the surgery with the old-style open surgery.

Cancer tissues are not cleaned better by touching and seeing directly with the hand?

This question is one of the most frequently asked questions to us. First of all, one of the important principles of cancer surgery is to operate without touching the tumor as much as possible. Laparoscopic surgery techniques are also performed in accordance with this principle and are not inferior to open surgery.

There are some essentials in the treatment of a serious disease such as cancer. Chief among these are

  • removal of diseased tissues within effective and wide safe limits (sometimes even sacrificing healthy ones)

  • safe and effective replacement of organ deficiency. Establishing a healthy configuration.

  • These two issues should never be compromised. And if there is the slightest doubt about providing these two conditions laparoscopically, the surgery is converted to open surgery.

Unfortunately, in some cases, some patients can be treated with this open surgery.

For which cancers is the most common type of surgery possible?

The most common cancers are colon and stomach cancers. It is done closed. If appropriate, laparoscopic surgeries can be performed even in liver and pancreatic cancers.

When robotic surgery is mentioned, it is as if a programmed robot performs the surgery. However, this is not actually the case. The surgeon still manages the instruments and mobility increases significantly.
