HOLEP is a method using laser technology in the surgical treatment of urinary duct obstructions due to benign prostatic hyperplasia. Although benign prostatic hyperplasia is not a cancer-type disease, it is a disease that causes frequent urination, intermittent urination, straining to urinate and even the need for catheterization, and seriously impairs the quality of life. While almost 40% of 60-year-old men have this disease, most patients are relieved with drug treatment.
How is the procedure performed?
The Holep procedure is a procedure that must be performed under sterile operating room conditions. It can be performed with the patient under general or regional anesthesia. It is entered through the urinary canal with a special endoscopic device. The prostate is reached with image guidance in the direction opposite to the direction of urine flow. Then, with laser energy, the enlarged parts of the prostate are separated from the prostate capsule and pushed into the bladder. Roughly speaking, if we compare the prostate to a tangerine with a urinary tract passing through it, slices of the tangerine are separated from its peel by laser and pushed back into the bladder. Then, with another special endoscopic tool, these tissues in the bladder are melted and removed from the urinary tract without any cutting. These tissues are sent to pathology specialist physicians to investigate whether they contain a cancer focus. The hospital stay of patients is generally 1 or 2 days and it is a very safe method. The urinary catheter inserted immediately after the operation is usually removed within 1 or 2 days. The main indicator for removing the catheter is to observe the urine clearly, without bleeding.
