Methods to obtain live sperm from men
The easiest method to obtain sperm from men is to collect semen by masturbation. In cases such as no sperm in the semen, retrograde ejaculation, inability to produce sperm, sperm may need to be collected using some methods.
Micro TESE (Obtaining sperm from testicle biopsy):
Azoospermia is the process of finding sperm or reproductive cells by making an incision in the testicles of men who do not have any sperm in their semen (affects 1% of men) using general or local anesthesia. It is performed by the urologist and pieces are taken from the testicle under a microscope without damaging the vessels and given to the embryologist for examination. The sperm search process can sometimes take 6-7 hours. MicroTESE has many advantages over normal TESE. Since it is done by viewing the vessels under a microscope, the damage to the testicle is minimal. There is also a large amount of difference between the amounts of tissue removed. The chances of finding sperm are similar.
Having no sperm in the semen can be either occlusive or non-occlusive. There is a higher chance of finding obstructive type sperm. Non-occlusive type is divided into 5 types. However, the chance of finding sperm is approximately 50%.
PESA (Percutaneous epididymal sperm aspiration):
It has a place in obstructive type azoospermia. It can also be applied if there is difficulty in ejaculating semen or if ejaculation is backwards (retrograde ejaculation). A small needle is inserted into the channels called epididymis on the testicles and semen is withdrawn. This semen is scanned for sperm.
MESA (Epididymal sperm aspiration under the microscope):
It is the removal of semen under microscopy by performing microsurgery from the epididymal channels on the testicle.
TESA (Testicular sperm aspiration):
It is a method of aspirating the semen by inserting a needle directly into the testicles without making an incision.
