Gynecomastia is a condition that causes psychological discomfort and disrupts the self-confidence of the person. Some men may restrict their activities (such as swimming, exercise) due to this condition. They may try to hide their big breasts by choosing clothes. Gynecomastia can be unilateral or bilateral. The common one is bilateral. Malignant tumors should be excluded, especially when unilateral. In gynecomastia, there may be excess fatty tissue or breast tissue. Sometimes there may be excess skin (especially due to weight loss).

Surgery and anesthesia
With gynecomastia surgery, breast size is reduced and the chest area is flattened. It is aimed to reveal the breast contours. It can be performed under local, general and sedation anesthesia. The type of anesthesia is decided according to the size of the breast tissue, the surgery to be performed and the patient's wishes. The type of surgery is decided according to the structure of the breast tissue. An idea about the structure of the breast tissue is obtained through ultrasound and examination. If the cause of gynecomastia is excess fat tissue, liposuction provides more benefits. Liposuction is the removal of excess tissue by entering several 0.5 cm scars with cannulas. The advantage of liposuction is that there are fewer scars and better contours are obtained. With liposuction, less bleeding and edema occurs and recovery occurs faster. The disadvantage is that in cases where there is excess mammary gland tissue, insufficient tissue removal may occur. Sometimes revision surgery may be required. If the cause of gynecomastia is excess mammary gland tissue and skin removal is necessary, surgery is performed with an open technique. The surgical scar is placed in a crescent shape on the border of the nipple with the normal skin. If skin removal is necessary, there will be no surgical scars. It is extended laterally on both sides of the sucking head. During open surgery, drains are generally used to reduce bleeding and fluid accumulation.
Post-operatively, dressings and bandages are used to reduce bleeding and edema and prevent sagging of the skin. Depending on the bleeding status, the drain is removed on the 1st or 2nd day. You stay in the hospital for 1 or 2 days. There may be edema and bruising in the first few weeks. After a few months, the shape of the breast will fully emerge. There may be pain in the chest area with arm movements for a few weeks.
Risks
As with any surgery, there are risks of anesthesia, bleeding and infection. There may be asymmetry between the two sides. There may be irregularities under the skin. There may be sagging of the skin. There may be a decrease or loss of nipple sensation.
