WHAT IS VAGINISMUS?
Vaginismus is not a disease. It is defined as the involuntary contraction of the muscles at the entrance of the vagina. If it is not resolved, it can be a nightmare for your marriage. Under the name of Vaginismus; One of the underlying reasons may be ANATOMICAL problems. THEREFORE: GYNECOLOGICAL EXAMINATION is essential for diagnosis. • Since spasms in vaginismus are completely involuntary, this situation is not the fault of the woman experiencing vaginismus. It is necessary to be PATIENT and UNDERSTANDING towards the patient.
WHAT ARE THE CAUSES OF VAGINISMUS?
Frequently;
• It is the wrong information given about sexuality in childhood and placed in the subconscious.
• Sexual abuse during childhood It may cause serious psychological problems. • The main cause of vaginismus is; It is FALSE, INCORRECT and INCOMPLETE information about sexual intercourse, which is subconsciously passed on from the family and the environment between the ages of 3-5, known as the OEDIPUS period when sexual identity is rapidly formed.
HOW IS VAGINISMUS DIAGNOSED?
• Failure to achieve sexual intercourse can be examined in two parts as organic and psychological reasons:
• Among the ORGANIC reasons:
1- Thick and hard hymen with high edges,
2- Congenital structural anomalies in the hymen and vagina; Reasons such as hymenal septum, vaginal septum, absence of vagina or short and blind vagina, vaginal infections, Bartholin cysts, painful vulvarvestibulitis can be counted among the organic causes.
Once these reasons are corrected with surgical or medical treatments, intercourse can be easily achieved. This situation is not considered vaginismus.
• PSYCHOLOGICAL reasons: BUT: WITHOUT A Gynecological PATHOLOGY: , *convulsion during intercourse,
*Wrong beliefs and attitudes, prejudices about sexuality,
*Especially Due to psychological reasons including sexual trauma such as rape at a young age, sexual intercourse cannot be achieved or even becomes impossible, which is the real state of vaginismus.
VAGINISMUS SHOULD BE DIAGNOSED BY A GYNECOLOGIST WHO HAS BEEN TRAINED ON THIS SUBJECT.
• However, patients who do not know this, based on hearsay information, seek relief from people who do not have medical treatment authority or knowledge, called pilgrims or mediums.
• Vaginismus progresses quite differently among people. Some women can have a gynecological examination but cannot have full sexual intercourse. A group of women do not accept any vaginal intervention. • Women who have vaginismus problems GENERALLY think that the hymen is very thick and that it forms a fleshy wall. HOWEVER, as a result of frequent gynecological examination, no problems with the hymen are found. If vaginismus treatment is delayed too much, conditions such as self-confidence, alienation from sexuality, erection problems, premature ejaculation may occur in men.
It has been determined that the inability to have intercourse is not due to a structural reason. After treatment, you may be diagnosed with "VAGINISMUS" by your doctor. • Vaginismus treatment is patient-oriented and should be customized depending on the degree of people's current problems. After you are informed about the severity of vaginismus and the treatment process, a special therapy plan is drawn up by your doctor. .Exercises specific to vaginismus, which can be done easily and painlessly, should be given to people as homework and they should be recommended to practice these exercises in sessions at home.
• III People who do not take these exercises seriously and do not repeat them at home cannot be successful. Muscle relaxants, pain-relieving creams, antidepressants, drinking alcohol, forcing sexual intercourse under general anesthesia, etc. It can never solve the problem.
The biggest problem with VAGINISMUS is that this problem is not shared with anyone. The basis of this problem lies in the fact that gynecological counseling was not received before marriage.
Based on my clinical experience: only 30% of women applying to the clinic can be diagnosed with real vaginismus. SO; In the other 70% group, the reason is generally superstitions and amateur first night stories, as well as sexual abuse experienced in childhood.
HOW IS THE EXAMINATION DONE?
• In the first examination, the external genital organs are evaluated for a few seconds only as observation and the degree of contraction of the vaginal muscles is determined. It is not possible for the patient to feel pain or discomfort during this examination. For this reason, people with vaginismus problems can be examined easily and without hesitation. • Patients with vaginismus during the examination; They can be divided into various classes, such as those that do not touch the labia minora and labia minora, those that do not touch the external genital organs but do not touch the vagina, and those that allow finger entry but do not allow speculum entry. Treatment sessions are determined according to these groups.
Sessions in VAGINISMUS treatment are approximately one hour. Treatment may last approximately one week. Treatments can be done consecutively or once a week.
