ACUTE SINUSITIS IN CHILDREN CURRENT DIAGNOSIS AND TREATMENT APPROACHES

Acute sinusitis cases remain important in the childhood age group.

Upper respiratory tract infection is one of the most common diseases in children. Symptoms of upper respiratory tract infection, which are mostly due to viral causes, are runny nose, fever and cough. Usually these symptoms are short-lived. If symptoms do not improve following an upper respiratory tract infection, the diagnosis of sinusitis should be considered.

Clinical symptoms of sinusitis;

There is a change in the diagnosis and treatment of acute sinusitis, which is common in children and often overlooked.

It is difficult to diagnose sinusitis in acute cases. Laboratory methods often do not help in diagnosis. Detection of the bacteria that causes sinusitis is only possible by entering the sinus head and taking a culture, which is extremely traumatic and is not recommended in children.

Another laboratory examination is radiological examination. Radiological examination alone is not diagnostic in cases of acute sinusitis in children, but it was performed to support clinical findings. Radiological examination has been abandoned in children with acute sinusitis. However, in cases of acute sinusitis, if there is a complicated situation, radiological examination can be performed. Considering that radiological examination is an invasive and costly approach, this decision seems to be rational.

Antibiotic treatment has been limited in cases of acute sinusitis in children. It is emphasized that the patient should be monitored without antibiotics, but antibiotic treatment can be started only in cases where the clinical picture deteriorates and sinusitis symptoms become severe.

In the selection of antibiotics, the appropriate antibiotic should be given at the appropriate dose.

In cases where there is no response to treatment, it is recommended to give high doses of antibiotics.

On the other hand, the thesis that recovery will be rapid in patients with antibiotic treatment is not accepted.

Nowadays, routine antibiotic application has been abandoned in cases of childhood acute sinusitis.

Additional treatment approaches;

These treatments can be applied in the first 3-4 days of the onset of the clinical picture. Its effects are limited.
