Our teacher, who has a comprehensive andrology training and deep experience in peyronie's surgeries, is a urologist. Dr. Coşkun Şahin answered frequently asked questions about peyronie surgeries.
What does Peyronie mean?
Peyronie is a penile curvature disease. It is characterized by curvature of the penis. In this disease, calcification (fibrosis) occurs in the sheath surrounding the penis. The penis curves forward, upward, to the right, or to the left. It loses its natural position. In addition, erection problems may occur. Not every penile curvature is Peyronie's. Some of our young patients have penile curvature as a congenital (congenital penile curvature) problem. Peyronie's is an acquired disease. It usually occurs after the age of 40. This disease is named after the French surgeon François Gigot de la Peyronie. François Gigot de la Peyronie was the first physician to describe this disease in 1743.
How to identify Peyronie?

Hardness felt under the skin of the penis
-
Skewing of the penis
-
Erection problems
-
Shortening of penis length

Who gets Peyronie's disease?
Peyronie's is a non-congenital penile curvature disease. As it is known, penile curvature can also come from birth. However, peyronie is an acquired health problem. It is mostly seen in the following groups:
-
Men over 40
-
Diabetic patients
-
Smokers
-
Patients with gout
-
Family history of penile curvature
Also, it is known that unconscious sexual relations and blows to the penis can cause Peyronie's disease.
Which side does the penis curve in Peyronie's disease?
Penis can curve right, left, up or down . However, we usually encounter an upward curvature.
How to measure the degree of penile curvature?
Men who suspect the curvature of their penis and feel a pain and hardening under the penile skin should definitely see a urologist. In this examination, the physician will examine the penis and determine the degree of curvature. To measure penile curvature The penis must be in an erect state. Patients who wonder how many degrees their penis is curved can also do this at home with the help of a protractor before consulting a doctor. However, for the healthiest and most accurate information, the patient must consult a physician.
Does penile curvature negatively affect sexual life?
Yes, it does. Penile curvature makes sexual intercourse difficult for both parties. If an erectile dysfunction is added to this, sexual life will be seriously affected. Therefore, it is necessary to treat penile curvature.
Does every Peyronie's patient have loss of erection?
Erectile dysfunction (loss of erection) does not occur in every Peyronie's patient. However, this problem comes to the fore in a significant portion of patients. We encounter erectile dysfunction in almost 1 out of every 3 peyronie's patients. This loss may occur with Peyronie, or it may have started before Peyronie. Diabetes is the most important risk factor here. Diabetes can trigger both peyronie's and erectile dysfunction. In addition, substance use, anxiety and depression can also cause these two diseases.
If the loss of erection occurred after penile curvature, this may not be a structural problem. It can be caused by the curvature itself and the psychological problems brought about by the curvature. The calcified tissue that causes the penis to curve can adversely affect the blood supply and erection of the penis. With the improvement of the curvature, the erection ability of the patient may also improve.
When should penile curvature be operated?
Surgical treatment in Peyronie's disease is performed 1 year after the onset of Peyronie's disease. This 1 year is the development process of the disease. During this time, we think that the curvature will reach its final measure. Again, during this period, drug therapy can be applied to reduce the curvature. Injection can be applied to the calcified tissue in the penis. Despite all this, if the curvature size has reached 40-50 degrees, the patient undergoes Peyronie's surgery at the end of the first year to straighten the penis.
How is penile curvature surgery performed?
There are two types of penile curvature surgery performed today. . The first is the correction of penile curvature with simple stitches placed on the penile tissues. This is a relatively simple operation. Approximately It takes 20 minutes. However, as a result of this surgery, the penis can be shortened by 1 cm. Another surgical method is surgeries performed with the help of grafts. In other words, tissues taken from another part of the body are transferred into the penis. This piece of tissue is usually taken from the inside of the mouth. If the surgery is performed with this method, the penis will not shorten, on the contrary, it may lengthen by 1-2 cm. This surgery takes about 2 hours. It should be done by urology physicians who have received the necessary training and experience.
Is every penile curvature treated?
If we are talking about non-congenital peyronie's disease, it is not always necessary to apply a treatment. If the measure of the curvature is below 20-25 degrees, if the person and his partner do not have any complaints related to sexuality, if there is no pain condition, not applying treatment and staying under follow-up is an option. There is no such thing as excessive bending and breaking of the penis. It does not turn into a fatal disease.
Does Peyronie's disease go away on its own?
Peyronie's disease has different prognosis.
-
Although it is very limited, some patients' penis may recover spontaneously after a period of bending. Such cases constitute approximately 10% of all peyronie's patients.
-
The penis is slightly curved. Afterwards, this curvature remains constant. Treatment is considered according to the extent of the curvature.
-
The curvature of the penis becomes dramatic. We often encounter 40-50 degrees of penile curvature. In this case, the person cannot have sexual intercourse. The penis can be curved up to 150 degrees. It is necessary to apply surgical treatment.
Patients also have pain in Peyronie's disease. However, this pain usually goes away on its own over time. Another problem caused by Peyronie's is the loss of erection. This is common in peyronie's disease. However, unlike pain, this condition does not go away on its own. It requires treatment.
How many degrees is the penile curvature normal?
Penis curvature is acceptable up to 20-25 degrees. In this level of curvature, difficulty in sexual intercourse or a loss of erection due to curvature is not expected. However, some men are dissatisfied with this curvature in their penis. They may withdraw from the relationship. In this case, it is important for our patients to get an opinion from a urologist and to hear from the doctor's mouth that this will not adversely affect sexual intercourse. However, at the request of the patient, it may be necessary to intervene in curvatures at this level in some special cases. On the other hand, if the curvature of the penis has reached 30-35 degrees, surgical treatment should be considered.
Does Peyronie repeat?
Yes, there is a risk of recurrence and bending of the penis again. What is important here is the type of surgery performed and the experience of the surgeon performing the surgery. As with every health problem, the patient's predisposition to the disease is another risk factor.
Can I have sexual intercourse after penile curvature surgery?
After the surgery, it is necessary to wait 3-4 weeks before having sexual intercourse. The surgeon who performed the surgery will tell you when you can have sexual intercourse.
