Gingivectomy and gingivoplasty operations are surgical treatment methods applied to ensure aesthetics in periodontal aesthetic surgery.
1- In the elimination of pseudopockets above the bone,
2- Providing aesthetics by eliminating asymmetries in the gums.
3- In removing fibrous and edematous growths on the gums.
4- It eliminates the form and shape deformities at the edges of the gums.
5- It corrects the cracks, craters and darkening of the gums.
6- It increases the retention of the crown.
7- Delayed passive eruption, which we call SHORT TOOTH SYNDROME. It is used to provide an aesthetic appearance by extending the length of the short teeth attached to the teeth. Roughly speaking, it can be called surgically reducing and shaping a certain amount of gum.
These surgical operations:
1- If the amount of attached gingival width is small.
2- If the bone under the gum is thick. &n bsp;
3- If the gingival pocket depths extend below the mucosa-gingival junction line.
4- If there was excessive tooth sensitivity before.
5- If oral care is poor.
6- It cannot be used if there are intra-bony defects, excessive inflammatory and edematous tissues. .
Advantages: Appropriate aesthetic results are obtained. The gingiva can be given the desired physiological shape. Gingival pockets can be completely eliminated.
Disadvantages: There may be some discomfort after the operation. The wound should be covered with a periodontal paste for about a week to protect it from external factors and trauma. Gum size. An undesirable amount of reduction may occur, especially if done by someone who does not have sufficient experience in this regard.
WHAT ARE GINGIVECTOMY TECHNIQUES?
1- Surgical gingivectomy.
2- Electrosurgery.
3- Cryosurgery.
4- Laser surgery.
5-Chemosurgery are used.
In my treatments of more than 40 years, surgical gingivectomy has been deemed the most appropriate and side-effect-free method. . Di Although other methods provide ease of practical application, they cause side effects such as bone necrosis, periodontal abscess, delay in wound healing, bone resorption, cement necrosis, and bad burning odor. .
WHAT IS TOOTH LENGTH EXTENSION? WHY DO IT? .
In order to improve the patient's smile, AESTHETIC CROWN LENGTH EXTENSION is performed for three basic reasons.
1- AESTHETICS; If more than 2 mm of gum is visible along with the teeth when the patient smiles and the teeth appear to be stubby in square form rather than rectangular, tooth lengthening surgery reduces the visible amount of gum and lengthens the length of the teeth. Usually this operation is performed in the upper front teeth area.
2- FUNCTION: In teeth with deep decay or fracture, the decay or fracture line may extend to the gum level or below. If a filling is desired, the filling may fall out because it is not possible to dry it well. If a crown is made, retention cannot be achieved due to short height, the glued crown often falls down and even breaks because the height is not enough.
3- REMOVAL OF GUM GROWTH: Some fibrotic, thick biotypes In people with this disease, the initial periodontal treatment does not provide complete recovery; hard gingival growths are excised and the length of the teeth is lengthened. If the pocket depth in the gingiva is above the bone and the gingival band is at least 4 mm, only gingivectomy and gingivoplasty surgery is sufficient to eliminate deformities. If the pocket depth is less than the amount the tooth wants to be extended, it is also applied to the alveolar bone surrounding the tooth. Sufficient distance is gained by performing FLAP OPERATION. The gum is removed, reductions are made from the tooth and bone surface, then the gingival edges are placed in the desired position. It is fixed to . Then, temporary crowns are often placed on the teeth, and after about 6 months, new aesthetic crowns are placed to give the patient an aesthetic appearance. It is essential to inform the patient before the treatment procedures, to have realistic expectations, to explain the possibility of restorative procedures after the surgical procedures and to obtain written approval regarding the cost. I wish you healthy days, sir.
