Question 1: The effects of facial aging can be explained by 3 mechanisms. These; 1- Loss or decrease of skin elasticity; 2- volume loss and 3- sagging.

The signs of aging are seen clearly and in a wide perspective around the eyes. If we examine them one by one:

  • Atrophy of the bones that make up the eyeball (structural shrinkage of the bones): This condition causes the volume of the eyeball to increase, which we call enophthalmos, and causes the eye not to be in the ideal position.

  • Volume loss: It is seen as a decrease in fat tissue, especially in the temporal (temple) region, upper eyelid and mid-face (cheek area). It may appear as tear trough, temporal and periorbital hollowing.

  • Loss of resistance of connective tissue and muscles over time will cause sagging. The traces of this are eyebrow sagging; eyelid drooping or drooping; tear gland sagging.

  • Loss of elasticity of the skin will cause an increase in skin laxity, skin sagging, thinning of the skin and wrinkles over time.

  • Question 2: Of course there is an ideal eye shape. It is defined as “almond”. The basis of some surgical procedures, which have been popularized under different names in recent years, dates back to ancient times, and these criteria are taken into consideration in eye area rejuvenation procedures.

    Beautiful eyes should first be symmetrical. The balanced distribution of the fatty tissue around the eyes and the fact that it is not reduced or displaced reflects a healthy and youthful look with more harmonious transitions. Having the skin around the eyes vigorous and taut makes it look bright and healthy.

    The upper and lower eyelids should have a soft fold and be able to cover the upper and lower borders of the cornea (limbus) to some extent. The upper eyelid fold should be 8-10 mm above the lash line. In the lower eyelid, this distance is 3-4 mm on the inner side and increases towards the outer side.

    In a young eye, the outer edge of the eye (lateral canthus) should be 2-4 mm higher than the inner edge (medial canthus). This appears at an angle of approximately 15 degrees. Outer and upward slanting eyes are a sign of youth.&nb sp;

    The eyebrow reaches its highest point on a perpendicular line drawn from the outer border of the cornea (lateral limbus). The lower eyebrow border is located 10 mm above the bone border (superior orbital rim). A vertical line from the outer edge of the nose wing (nasal ala) upwards marks the inner border of the eyebrow; A line drawn along the inner and outer borders of the eyebrow forms the length of the eyebrow, and a line drawn from the outer edge of the nose to the outer border of the eyebrow forms the hypotenuse of the right-angled triangle. In addition, the distance between the outer edges of both eyebrows should be approximately 2 times the height of the forehead. These data may show ethnic differences, and although they are accepted as beauty measures, the understanding of beauty is too subjective to comply with mathematics or criteria.

    Question 3: Within my own practice, it would not be right for me to give a clear answer to this question in a general way. However, I can say that in my practice, my non-surgical procedures, especially for the eye area, have increased.

    With the use of a mask, we can think of the fact that only our eyes are exposed and other parts of the face are not visible, drawing attention to the eyes and their surroundings. However, the eye is always the focal point that attracts attention with its expressions and movements. The poems, songs and sayings written about him tell us that the interest in the eyes is independent of the mask. Finally, if we end this question with the beautiful words of Audrey Hepburn, "The beauty of a woman is seen in her eyes, because the eyes are the entrance to the heart, where love lives."

    Question 4: Deep wrinkles, drooping eyebrows and eyelids, and tear trough deformity are some of the conditions that cannot be covered with make-up.

    Question 5: Procedures such as Botox, filling, mesotherapy and laser are applications that can be performed without surgery.

    Question 6: These procedures, also called minimally invasive, are applied to patients with milder complaints and in the young and middle age group. can be applied to individuals. It can also be applied to older people with more severe deformities, at the request of the person, with the understanding that the result may be suboptimal.

    These are short procedures that can be performed in the examination room of the hospital. Redness may occur at the application site, but the person can continue his normal daily life.

    Botox applications last between 3-5 days. which begins to show and has a lifespan of approximately 4-6 months. The effect of filler applications can be seen immediately and will be effective within 6-12 months, depending on the cross-links and weight of the filler. The effects of mesotherapy applications begin to be seen a few days after the procedure, but repeat sessions will be required at intervals of 7-15 days to be more beneficial. Afterwards, it can be applied again at certain intervals.

    The light that Botox prevents wrinkles will reflect directly and the skin around the eyes will look lively and bright. It will also give a more vigorous appearance by preventing sagging of the eyebrows and eyelids, albeit minimally.

    Filling prevents the hollowing of the temple area, tear trough deformity, and the more pronounced bone structure due to volume and displacement in the fatty tissue, thus making the patient look alive and healthy.

    Mesotherapy will give a more vibrant, brighter and healthier appearance by providing the vitamin support the skin needs.

    Question 7: The upper eyelid sags with aging, directly affecting the visual field. In other words, although there is no loss of the visual function of the eye, it creates a physical obstacle. The feature is the view from the outside. Additionally, the most common complaint we receive is that it creates a tired appearance. Drooping eyelids may also be accompanied by drooping eyebrows. Such conditions will be revealed during the examination and can be performed together with eyebrow lift, forehead lift or temple lift procedures.

    These complaints can be applied to the current middle-aged and older patient group.

    The surgery is minimally painful, will last approximately 30-60 minutes, and can be performed with regional anesthesia. It does not require hospitalization. There may be edema and bruising in the eyes, these complaints will disappear within 1 week. Although its effect is seen immediately, it may reach its final state within a few months, depending on the time it takes for the edema to subside.

    Since aging will not stop after the surgery, its effect will last for 8-10 years.

    Question 8: You can get younger and livelier looks. You can get rid of the tired expression on your face. You can make eye makeup that is more visible and looks better.

    As important as choosing the right technique and physician is, it is important to be clear about any accompanying diseases or other special conditions in the pre-operative evaluation. It should be conveyed to your doctor. This surgery may not even need to be performed, especially in people with dry eyes. After the surgery, the eyes may remain open for a while while sleeping. This is a temporary situation, but it is important to keep the eye moist and use tear gels during this period.

    After the surgery, it is important to apply ice, lie with your head elevated, and use the medications prescribed by your doctor regularly.

    Question 9: Partial removal of excess skin for the lower eyelid, similar to the upper eyelid, is necessary for the outer eye. Suspension of the edge (cantoplasty) and removal of excess fat herniation under the eye are performed together. A midface lift can be combined with a face lift or other surgical procedures. It may be beneficial to perform it under general anesthesia.

    Question 10: In order to camouflage under-eye bags, oil or ready-made fillers can be applied around them. The main treatment is to perform lower eyelid plastic surgery, and in this procedure, the herniated under-eye fat tissue can be removed or excess fat tissue can be transferred to the area called tear trough deformity.

    Question 11: Dark circles around the eyes are a factor that negatively affects the quality of life. They create complaints. Although its etiology is not fully understood, pigmentation may be due to structural and vascular problems. One of the main reasons is the accumulation of melanin pigment. Additionally, decreasing skin thickness with age can cause the veins to appear darker, making them appear darker. Apart from these, it may be caused by fat herniation and chronic skin disorders in that area.

    Laser methods, chemical skin peeling or topical creams can be used to lighten the color. In cases of volume loss, fillers and fat grafting can be applied. PRP and mesotherapy agents have been shown to be effective.

    If it is due to fat herniation, the herniated fat tissue may need to be removed along with lower eyelid aesthetics.
