What is vaginismus?
Vaginismus; It is a condition in which sexual intercourse becomes impossible or painful as a result of the contraction of the muscles surrounding the outer part of the vagina during sexual intercourse. Contractions in the vagina may also be accompanied by contractions in other parts of the body. A woman who panics due to the intensity of contractions and fear may push her partner away during intercourse and completely close herself off from the relationship. These behaviors may be perceived by the woman's partner as if she does not want the relationship or is consciously shutting herself off. However, these contractions are completely involuntary and it is not in the woman's power to relax.
The name of the muscle group in vaginismus isPC (Pubococcygeus)also known as the love muscles group. These muscles draw a line from the urethra to the anus and play a role in many bodily functions, from urination to birth. Under normal conditions, these muscles have the ability to stretch considerably during sexual intercourse. If the person does not experience fear and anxiety, and if the desire and arousal are sufficient, the muscles stretch according to the size of the penis and sexual intercourse can easily occur. We can compare the muscle structure to a pleated skirt; it takes the shape of the structure it contains. In women with vaginismus, the condition occurs with an involuntary vaginal reflex and spasms narrow the entrance to the vagina.
Women with vaginismus think that their vagina is too small to accommodate the penis or that their hymen is too thick and they will have great difficulty in sexual attempts. There is no physical obstacle in the body of these women to experience sexuality, it is rather their faulty mindset. In a sense, we can call vaginismus a panic attack of the vagina. In other words, it is related to the person's cognitive and psychological processes rather than physiological processes.
Vaginismus types
Vaginismus problem can be primary and secondary.
Primary vaginismus: It has existed since the person became sexually active.
Secondary vaginismus: It is the subsequent occurrence of contractions in a woman who previously had painless sexual intercourse. This condition mostly occurs after a trauma or surgical intervention.
What is the cause of vaginismus?
Vaginismus is not a very rare condition. It is a psychological problem. Faulty thought structures and negative conditionings settled in the person's mind cause this problem. The reasons for this are lack of knowledge, insufficient sexual education, exaggerated expressions of women about their first night experiences, being raised with strict moral values, perceiving sexuality as shameful, sinful, dirty, and hearsay misinformation about the hymen or sexual intercourse. Being under the influence, high level of anxiety, fear of pregnancy, negative sexual experiences, bad childhood experience, problematic parent-father relationship, over-controlled personality traits, emotional attachment difficulties, lack of trust and communication problems with the spouse are common reasons.

Vaginismus should not be thought of as a problem experienced only by women. Men also have an important role in experiencing and treating the problem. For this reason, the man should not leave his wife alone and the spouses should participate in the treatment together. What is important for treatment is that cooperation and harmony continue between the spouses.

In vaginismus, cognitive-behavioral therapy method, various breathing and relaxation exercises, and practices given together with the couple form part of the treatment. During the initial process of treatment, the couple's or individual's lack of sexual knowledge or incorrect learning is evaluated. Sexual education is given to address these deficiencies. It is possible by addressing both the mind and body. The aim is to clear the mind of negative conditioning and anxiety regarding sexuality and to make the body accept and enjoy vaginal penetration.
Mechanically solving vaginismus is not sufficient for treatment. Two stages are followed in the treatment to offer the couple a healthy and happy sexual life. In the first stage, we aim to have sexual intercourse. In the second stage, techniques for enjoying sexual intercourse and achieving orgasm are taught.
It can be treated with any medication or operation.
